Universal health coverage (UHC) is where all people have access to the health services they need, when and where they need them, and without financial hardship. UHC is an objective shared by countries worldwide. Many low- and middle-income countries (LMICs) have made progress towards UHC, but large challenges remain (owing to COVID-19, the current war in Ukraine, climate change related natural disasters, and other crises). At the same time, the COVID-19 pandemic has highlighted at the global level the social and economic importance of health and UHC. There is, therefore, a critical opportunity for change that we embrace in this Unit: to strengthen progress to UHC in such challenging times, using the tools and perspectives of health financing and economics. The Unit will use these tools and perspectives to carry out in-depth, policy-relevant analyses on specific health finance-related topics, engage with communities and stakeholders, and conduct capacity building. The Unit’s research will comprise the analysis of existing data, modelling, primary data collection and policy evaluation. This will be shared across four policy themes and four cross-cutting methodological areas. The policy themes revolve around a set of national and international challenges identified through extensive stakeholder consultations: (1) raising money for health; (2) allocating money for health; (3) ensuring healthcare access for left-behind groups; and (4) the role of governance, including the interaction between the health system and other sectors. The methodological areas will feed into the policy themes by ensuring the application and development of rigorous, innovative methods to address the challenges identified. They cover (1) methods of evaluating the impact of policies, (2) the analysis of distributional (“equity”) effects, (3) ways of assessing value-for-money of the policies in question, and (4) aspects of political economy. The collaborative research programme builds directly on the work of the University of York’s NIHR Global Health Econometrics and Economics Group (GHE2), which focussed upon Brazil, Indonesia and South Africa. For the Unit, India is included as an additional research location. These four ‘middle-income’ countries comprise nearly two billion people and 45% of the world’s population living in absolute poverty. The countries are characterised by substantial within-country economic and health inequalities, hosting large marginalised and left-behind populations (e.g. geographically-isolated populations, slum dwellers, socially marginalised castes, and ethnic or religious minorities). Understanding how to advance UHC in these contexts, and among diverse vulnerability profiles, provides significant opportunities for mutual, between-country learning and knowledge spill-over benefits to other low- and middle-income countries. The long-term aim of the Unit is to strengthen countries’ ability to adapt health financing policies and programmes for resilient, equitable and sustained progress towards UHC. It will be achieved by the production of rigorous evidence on the key health financing and resource allocation challenges. TAG( 1 ) 1) Improve health and reduce deaths associated with diabetes, heart and lung diseases in people with severe mental illness (SMI) by addressing the most common health risk behaviours identified by a physical health survey, assembling a cohort of people who can be approached for future research and by designing interventions aimed at changing health risk behaviours (tobacco, physical activity, alcohol and diet) based on a brief talking therapy. 2) Reduce depression and anxiety in people with chronic physical health conditions by integrating the detection and treatment of depression/anxiety into an intervention package for chronic diseases delivered in primary care. 3) Build research teams and capacity to secure competitive funding and delivery world class applied health research informing policy and practice in these areas. Following a costed extension, additional objectives will include: Work stream 1: Conduct workshops with a range of stakeholders to understand facilitators and barriers to integrating services for identifying and managing depression in tuberculosis (TB) and chronic hepatitis services. 1. This will be based on a relatively simple psychological talking treatment called Behavioural Activation (BA). The projects objectives are:1. Tobacco use is a long-term condition in itself, which can also lead to the development of other long-term conditions. It is one of the biggest preventable factors responsible for health inequality but remains unaddressed among people with serious mental illnesses who live in low-resource settings such as South Asia. Our team has several years of experience in working with people who have serious mental illness and have made significant advances in recent years in the UK and South Asia. We have conducted research into the causes of health inequalities for people with serious mental illness in both settings. We have established a shared understanding of the nature of this problem under an initiative to build sustainable global collaborations. In South Asia, we have made advancements under our IMPACT Global Health Research Group (now a Centre) by developing a culturally-appropriate behavioural treatment to help people with serious mental illness in India and Pakistan to quit smoking. In the current programme, we will build on our previous research and experience, extending to three South Asian countries (Bangladesh, India and Pakistan) with a high prevalence of tobacco use. We will adapt the previously developed treatment to cater to a wider group of individuals, including those who live in Bangladesh as well as those who use smokeless tobacco. We will then test this treatment in a large clinical trial (‘a fair test’) to see if offering tobacco cessation works and is value for money. Conducting a large trial is a complex undertaking, and we have significant expertise in this area. Our team includes experts from a range of backgrounds, as such research requires a range of perspectives. We will use valid and reliable tests to assess the success of our treatment in helping people to quit tobacco. We will also find out which strategies are useful in delivering tobacco cessation in mental health facilities. Alongside the research, we will develop expertise in conducting research in mental health, train clinicians to deliver tobacco cessation treatments and support policymakers in using our research findings in practice. By delivering hypertension care through community pharmacies, the team seeks to enhance access to care and reduce the risk of cardiovascular diseases (CVDs).CVDs are the leading cause of death globally, and hypertension is the most significant modifiable risk factor for CVDs. In Bangladesh and Pakistan, a high percentage of individuals with hypertensionremain undiagnosed, untreated, or have uncontrolled blood pressure, particularly among low-income urban residents who face significant barriers to accessing healthcare. Community pharmacies are widely accessible and frequently used by these populations but are not fully integrated into formal healthcare systems.The long-term aim of the award is to reduce premature deaths and disabilities caused by hypertension and associated CVDs in Bangladesh and Pakistan. This intervention will enable community pharmacists to provide hypertension care, including blood pressure monitoring, medication adherence support, health education, and referrals to primary care physicians.Commencing in March 2025, the award will map existing community pharmacies and engage with key stakeholders to understand the current practices and identify barriers and facilitators regarding the implementation of the intervention. The intervention will be tested via a feasibility study before undertaking a randomised control trial to assess the effectiveness and cost effectiveness of the intervention in reducing systolic blood pressure. 1. Develop a context-specific intervention aimed at managing hypertension among low-income urban residents in Bangladesh and Pakistan through community pharmacies.2. Explore the feasibility of delivering the intervention and refine methods and procedures for a large-scale trial.3. Assess the effectiveness of the intervention in reducing systolic blood pressure among low-income urban residents aged 30 years and older with hypertension in Bangladesh and Pakistan.4. Conduct a comprehensive process evaluation to understand the implementation process of the intervention and how the health system and urban context in each site influence its effectivenessand sustainability.5. Develop a cost-effectiveness model for hypertension care and assess the long-term cost-effectiveness of the intervention, including the costs of scaling up and the value of investing instrategies to improve uptake.6. Enhance capacity in applied research, leadership, and community-based hypertension management among researchers, pharmacists, and healthcare professionals in Bangladesh andPakistan.7. Establish Community Advisory Panels in each country to engage patients, families, and community members in the research process.8. Work with key stakeholders to translate our project findings into policy and practice and enact plans to sustain research and its impact.9. Disseminate the intervention findings through academic publications, policy briefs, workshops, print and social media, and community events.
