XM-DAC-6-4-013244-05-0 AICS - Italian Agency for Cooperation and Development EmPoWeReD II aims to expand the initial EmPoWeReD project (AID 12590) to 14 new regions in Tanzania and Zanzibar. This phase focuses on extending vocational training, promoting workers' rights and introducing new activities to prevent child trafficking and child labor in the domestic sector, while supporting safe migration. The project will be extended to regions with VETA centers to provide vocational training to domestic workers and will collaborate with CHODAWU to strengthen workers' rights and combat trafficking. The initiative aims to transform paid domestic work from an informal sector to a recognized and formalized type of work across Tanzania, supporting the ratification of ILO C189, promoting dialogue on C143 and addressing gaps in bilateral agreements to ensure conditions of decent work for migrant domestic workers. In Tanzania there are an estimated 883,779 domestic workers and another 203,662 in Zanzibar, many of whom are girls or women who started working as children. The project aims to have a direct impact on 5% of the total domestic workers in the country and to safeguard minor girls. EmPoWeReD II will target an additional 14 regions, bringing the total to 20 out of 24 regions (83%), with full coverage in Zanzibar. Interventions on trafficking in persons (TIP), in particular of minor domestic workers, will cover 17 regions where trafficking is prevalent. This second phase capitalizes on the results already obtained to promote the national adoption of decent work standards and professional qualifications for Domestic Workers. The overall objective is to ensure equal access to technical, vocational and tertiary education by 2030, in line with SDG target 4.3. In line with the Sustainable Development Goals (SDGs), in particular with Goals 4.3, 4.4, 8.5, 8.7 and 8.8, the project focuses on improving professional skills, education, working conditions and trade union support for domestic workers, while addressing the issue of trafficking, as outlined by ILO Convention 182, contributing to the reduction of poverty. The target group includes domestic workers and vulnerable minors (aged 15-24) involved in domestic work. EmPoWeReD II focuses on three key outcomes: 1. Access to professional training and job opportunities for domestic workers and child victims of trafficking in VETA centers in the target regions (SDG objectives 4.3 and 4.4). 2. Increase in the economic-union support association network in the target regions of domestic workers for greater formalization of employment (SDG Objectives 8.5 and 8.8). 4. Counteracting the trafficking of minors linked to domestic work through the implementation of appropriate policies and regulations by promoting access to training courses (SDG Objective 8.7). The partnership is based on specific skills and professional expertise, constituting a well-defined multi-sectoral response to the challenges faced by domestic workers, both nationally and internationally. ICC Integrated Community Centre, an NGO supporting migrant women in Kuwait with branches in Bahrain, Jordan, Lebanon and Qatar, already cooperates with CVM in Ethiopia to assist Ethiopian migrant women who are victims of abuse. The ICC will highlight the challenges faced by domestic workers in MENA countries to national authorities and PEAs, advocating for safer working conditions. DOMINA adds value in training VETA teachers and promoting higher international standards. It promotes the responsibility of PEA and the union in protecting female workers, drawing on their Italian experience in the training and job placement of domestic workers. CHODAWU, the reference union in Tanzania and Zanzibar, has a central role in building the protection network, while WOTESAUA specializes in the protection of minors. IDWF will provide expert trainers on global issues such as the implementation of C189 after ratification, MENA experiences, “wage theft” and other global challenges faced by members around the world. In linea con gli Obiettivi di sviluppo sostenibile (SDG), in particolare con gli Obiettivi 4.3, 4.4, 8.5, 8.7 e 8.8, il progetto si concentra sul miglioramento delle competenze professionali, dell’istruzione, delle condizioni di lavoro e del sostegno sindacale alle lavoratrici domestiche, affrontando al contempo il tema della tratta, come delineata dalla Convenzione 182 dell’ILO, contribuendo alla riduzione della povertà. Il gruppo target comprende lavoratrici domestiche e i minori vulnerabili (di età compresa tra 15 e 24 anni) coinvolti nel lavoro domestico. EmPoWeReD II si concentra su tre risultati chiave: 1. Accesso a formazione professionale e opportunità lavorative per lavoratrici domestiche e minori vittima di tratta nei centri VETA delle regioni target (obiettivi SDG 4.3 e 4.4). 2. Incremento della rete associativa di supporto economico-sindacale nelle regioni target delle lavoratrici domestiche per una maggiore formalizzazione dell'occupazione (Obiettivi SDG 8.5 e 8.8). 3. Contrasto della tratta dei minori legati al lavoro domestico attraverso l'implementazione di politiche e regolamentazioni appropriate favorendo l'accesso a percorsi di formazione (Obiettivo SDG 8.7). La partnership si basa su competenze specifiche e competenze professionali, costituendo una risposta multisettoriale ben definita alle sfide affrontate dalle lavoratrici domestiche, sia a livello nazionale che internazionale. Minors often do not access treatment, rehabilitation or education services. The specific objective is to improve the effectiveness and inclusiveness of the food and nutrition security system of the communities living in the Dodoma region, particularly in the districts of Kongwa, Chamwino and Bahi, with a particular focus on the most vulnerable groups (children, pregnant women and/or women heads of household). This objective can be achieved through the following expected results in response to the needs highlighted in the needs assessment: 1. Increased access to quality severe acute malnutrition services in the districts of Bahi, Kongwa, Chamwino and at the Dodoma Regional Referral Hospital (DRRH). 2 Increased awareness, knowledge and adoption of good hygiene-nutritional, agricultural and food diversification practices by communities, particularly by women of reproductive age and caregivers of children under 5 years of age 3. Increased Knowledge, Capacity of Producer Organisations and Market Access for Sustainable Horticulture Products. With a total investment of €1,894,150, the direct beneficiaries of the initiative are 114,000 people, mainly in rural areas (about 85%), as follows about 21,800 children under 5 years of age screened for malnutrition about 22400 women and caregivers of children under 5 years of age, sensitised on good hygienic-nutritional practices and gender equity 42,000 farmers of whom more than 25,200 are young people engaged in productive diversification activities About 3,000 farmers are supported in capacity building and market access for indigenous vegetables (AIV) and more than 20,000 consumers in urban areas, improve the possibility of making healthy and informed choices. In Tanzania, in un contesto di rallentamento della riduzione della povertà e di aumento delle diseguaglianze il Covid-19 funge da acceleratore di una crisi che in primo luogo è del sistema alimentare prima che sociale e nutrizionale. Nella Regione di Dodoma, al centro del Paese occorre affrontare la doppia sfida della malnutrizione acuta (0,4%) e cronica (37.2%) ancora inaccettabilmente alta in un contesto rurale il cui cambiamento è trainato dall’effetto di crescita della capitale, con opportunità di sviluppo che tardano a riverberarsi sulla Regione. L’ineguaglianza in termini di sicurezza alimentare grava particolarmente sulle donne, elemento paradigmatico di una situazione di vulnerabilità, di limitato accesso a risorse, conoscenza e capacità nella società locale. Tuttavia cominciano a manifestarsi i segnali di una svolta, che la stessa pandemia ci obbliga a intraprendere per attuare una trasformazione dei sistemi produttivi e sociali in una logica di transizione agroecologica. L'orticoltura è il settore agricolo che meglio incrocia il dinamismo della domanda agricola, le potenzialità di impiego per piccoli produttori in particolare donne e giovani (oltre il 70% in Tanzania) di diversificazione agricola e integrazione della dieta.. Inoltre l'orticoltura non è solo un'attività familiare ma è anche il cuore della coesione sociale della comunità attraverso la cooperazione intergenerazionale. Diverse specie trascurate e sottoutilizzate, tra cui gli ortaggi indigeni africani (AIV) e i legumi, sono coltivate in un contesto di riduzione del rischio con basso impiego di input esterni, di strategie economiche familiari combinate e modelli di consumo tradizionali. Ma queste colture sono anche parte del patrimonio culturale nazionale e la diversità biologica e culturale nelle mani dei contadini è una risorsa per affrontare contemporaneamente le sfide della salute, della sicurezza alimentare, dell'ambiente e dell'economia. Per questo si uniscono allo sforzo l’associazione LVIA, attiva nel settore acqua e agricoltura, Medici con l’Africa CUAMM eccellenza nel Paese per il supporto ai sistemi di salute e contrasto alla malnutrizione, la Sokoine University of Agricolture che ha sviluppato conoscenze e competenze sull’agricoltura nutrition sensitive che fa leva sugli AIV e MVIWATA la principale rete di contadini del Paese, in grado di promuovere e sostenere il cambiamento. In termini più strutturati l’Obiettivo specifico è il miglioramento di efficacia e inclusività del sistema di sicurezza alimentare e nutrizionale delle comunità residenti nella regione di Dodoma in particolare nei distretti di Kongwa, Chamwino e Bahi con un particolare focus sulle categorie più vulnerabili (minori, donne gravide e/o donne capofamiglia) Tale obiettivo può essere raggiunto attraverso i seguenti risultati attesi rispondenti ai bisogni evidenziati nel needs assessment: 1. Furthermore, a pivotal component of the intervention will be the creation of meaningful linkages with the private sector through private-public partnerships platforms, dual and apprenticeships schemes, including circular mobility between Tanzania mainland and Zanzibar Archipelago. The intervention will also leverage on the strong presence of the Italian companies in the HO.RE.CA industry based in Zanzibar. Finally, the capacity building of government authorities on sustainable tourism will promote a more responsible, sustainable and diversified sector. This part of the population has no access to adequate basic services which could improve their living condition, especially with regard to their health (SDG3) and education (SDG4). The need assessment shows that: 1. The number of children with disability is not dropping. The services to prevent childhood disability during antenatal and postnatal period are inadequate, ineffective, not accessible for mothers and young women. 2. There is no system of disability registration or certification for children aged between 0 and 4 years. The health facilities’ staff makes early diagnosis very rarely. Only 3,4% of children access to Community Rehabilitation Programme. 3. In Tanzania, more than 340.000 children with disability stay out of the scholastic system due to inaccessible school facilities and scholastic staff not trained adequately. The project ‘We care’ aims to activate a multisector programme to promote access to high-quality services (prevention, diagnosis, health, community based rehabilitation and socio-education inclusion) for children and youths with disability (0-15 years old) and their families in the following provinces: Iringa Rural, Mufindi and Kyololo (Iringa Region-Tanzania). In particular, the project uses the CBR (Community Based Rehabilitation) methodology. The lead applicant, L’Africa Chiama (AC), has a decade experience in the use of this methodology. CBR is not a treating method or a procedure. This strategy attempts to solve issues faced by children with disability activating a network of people that can be useful and can participate to the effort of a community which is trying to improve the quality of life of the child with disability. The CBR assumes that the community hast to be involved in the rehabilitation process directly. Only taking the responsibility of the issues that affect the children with disability, the community can completely understand their needs. With reference to this methodology and in accordance with local authorities, the lead applicant and the partners –AC, IBO ITALIA, CO.P.E, Shivyawata Tanzania Federation of Disabled People’s Organisations and Health Centre Nyololo (KCA)- have considered essential to take action in the following sectors: R1: Prevention and Health. The project aims to train health staff, strengthen health facilities involved, sensitize local community in preventing risk factors that influence the occurrence of functional limitation such as congenital malformation, rare disease, disease arisen during the pregnancy and so on. R2 Health and CBR. The project attempts to activate a referral programme and to focus on the care of children with disability thanks to: certification and register system of disability, training of health facility personal on early diagnosis and referral, activation of CBR programme - thanks to training and empowerment of children with disability’s families. R3 Inclusive Education. The project intents to promote the access of children with disability to a high-quality education through: specific trainings for teachers, the removal of architectural barriers and activities of inclusive sport. In addition, the project focuses on the importance of sensitize the community. This is fundamental to create a community that speaks out and asks for high-quality services for fragile people at risk of margination and by doing so, contribute to improve life condition of the society as a whole. In Tanzania secondo i dati nazionali il 9,3% della popolazione totale presenta una forma di disabilità e fra questi più della metà sono minori. Un’ampia fascia di persone che non ha accesso ad adeguati servizi di base che possano migliorare le loro condizioni di vita, soprattutto in riferimento alla salute (OSS3) e all’istruzione (OSS 4). Dal need assessment effettuato emerge infatti che: 1. il numero dei bambini disabili non è in calo e che i servizi di prevenzione della disabilità infantile nel periodo prenatale e postnatale non sono sufficienti, efficaci ed accessibili per le madri e le giovani donne; 2. non esiste alcun sistema di certificazione e registrazione della disabilità per i bambini compresi nella fascia 0-4 anni, il personale sanitario raramente effettua diagnosi precoci e che solo il 3,4% dei bambini ha accesso a programmi di riabilitazione su base comunitaria; 3. in Tanzania oltre 340.000 bambini con disabilità rimangono fuori dalla scuola a causa di strutture scolastiche non accessibili e carente formazione del personale scolastico. ll progetto WE CARE intende attivare un programma multisettoriale volto a favorire l'accesso a servizi di qualità (prevenzione, diagnosi, salute, riabilitazione su base comunitaria e inclusione socio educativa) da parte di bambini e ragazzi con disabilità (0-15 anni) e delle loro famiglie nelle Province di Iringa Rural, Mufindi e Kyololo (Regione di Iringa - Tanzania). In particolare il progetto utilizza la metodologia CBR (Community Based Rehabilitation - Riabilitazione su Base Comunitaria), su cui il soggetto proponente, L’AFRICA CHIAMA AC, vanta una decennale esperienza. La CBR non è un metodo di trattamento o un protocollo , ma una strategia che partendo dalla conoscenza dei problemi del disabile cerca di dare una soluzione agli stessi avvalendosi della catena solidale di quanti si sentono in grado di essere utili e si fanno partecipi degli sforzi che una comunità di persone attua per migliorare la qualità di vita del disabile stesso. La Riabilitazione su base comunitaria parte dal presupposto che la comunità deve essere direttamente coinvolta nel processo riabilitativo: solo se la comunità si fa carico in prima persona delle problematiche delle persone disabili ne può comprendere appieno i bisogni. In riferimento a tale metodologia i soggetti coinvolti nella proposta, IBO ITALIA, CO.P.E., Shivyawata Tanzania Federation of Disabled People's Organisations e Health Centre Nyololo - KCA , in accordo con le autorità locali, hanno individuato come necessari i seguenti settori di azione: R1: Prevenzione e salute. Un programma che prevede la formazione al personale sanitario, il rafforzamento delle strutture sanitarie coinvolte e la sensibilizzazione alla comunità locale per prevenire i fattori di rischio per l’insorgenza di limitazioni funzionali (malformazioni congenite, malattie rare, malattie durante la gestazione, etc.). R2: Salute e CBR. Un ampio intervento che intende attivare un programma di referaggio e cura del bambino con disabilità fra 0-4 anni attraverso: un sistema di certificazione e registrazione della disabilità, formazione al personale sanitario sulla diagnosi precoce e referaggio, attivazione di un programma di riabilitazione su base comunitaria attraverso la formazione e l’empowerment delle famiglie dei bambini. R3: Educazione inclusiva. Azioni che favoriscono l’accesso da parte dei bambini disabili ad un’educazione di qualità attraverso l’attivazione di percorsi formativi per gli insegnanti, l’abbattimento di barriere architettoniche e lo sport inclusivo. The direct beneficiaries of the initiative are 800 minors and young adults with disabilities, with a special focus on those with epilepsy and malnutrition problems, residing in the regions of Mbeya, Morogoro and Dar Es Salaam; 280 mothers of children with disabilities; 28 rehabilitation health workers; 340 doctors and health workers; 153 students with disabilities attending Yombo and VETA centers, 215 teachers and staff of the training centers who will be trained thus obtaining new important skills, 30 employers who will be sensitized on job inclusion and who will host in apprenticeship the students trained by the centers, 120 officials and representatives of governmental and non organizations in Tanzania. L’iniziativa progettuale ha come obiettivo generale l’inclusione sociale delle persone con disabilità in Tanzania, il loro inserimento lavorativo, il miglioramento dell’accesso ai servizi sanitari, il potenziamento dei programmi di alimentazione e la loro piena partecipazione, come cittadini, alla vita delle proprie comunità in coerenza con la metodologia della CBID (Community Based Inclusive Development). I beneficiari diretti dell’iniziativa sono 800 minori e giovani adulti con disabilità e in particolare quelli con problemi di epilessia e malnutrizione, residenti nei territori delle Regioni di Mbeya, Morogoro e Dar Es Salaam; 280 caregiver di bambini con disabilità; 28 operatori sanitari della riabilitazione; 340 medici e operatori sanitari; 173 studenti con disabilità presso i centri Yombo e VETA (Vocational Education and Training Authority), 215 tra insegnanti e staff dei centri di formazione che verranno formati ottenendo così nuove importanti competenze, 30 datori di lavoro che verranno sensibilizzati sull’inclusione lavorativa e che accoglieranno in apprendistato i ragazzi formati dai centri, 120 funzionari e rappresentanti delle Istituzioni governative e non- tanzaniane. Il progetto si svilupperà su tre linee di azione complementari. 1. Una prima azione si rivolgerà ai minori disabili con problemi di epilessia e malnutrizione (spesso correlati nel circolo “vizioso” povertà - disabilità – malnutrizione). Saranno avviati 2 ambulatori per la diagnosi e cura dell'epilessia presso i Centri di Riabilitazione Simama - Mbeya, e l’Ospedale St. Francis di Ifakara. Sarà inoltre promosso uno screening sanitario, per identificare quei bambini con disabilità con problemi di malnutrizione che afferiscono ai Centri di riabilitazione Simama CBR di Mbeya e “Antonia Verna - Kila Siku” di Dar, e sarà organizzato il supporto nutrizionale intensivo con fornitura di farine e preparati per i piccoli pazienti. Saranno inoltre avviate azioni formative per e di sostegno ai caregiver sui temi della malnutrizione e della epilessia. È prevista una strategia di formazione e sensibilizzazione contro lo stigma in riferimento a epilessia e disabilità, che coinvolgerà almeno 2 milioni di cittadini tanzaniani. Sarà promossa la formazione di 20 dirigenti medici provenienti da 10 regioni della Tanzania presso l’Università SFUCHAS di Ifakara sulla diagnosi e cura dell’epilessia. I dirigenti formati si faranno poi carico della formazione, sulla diagnosi e cura dell'epilessia, di 320 medici e operatori sanitari presenti nei rispettivi territori di origine. 2. Una seconda linea d'azione prevede di affrontare alcune delle principali barriere che impediscono alle persone con disabilità (PWD) di accedere al mercato del lavoro, limitando così le loro possibilità di inclusione sociale ed economica. La componente in questione sarà affidata al partner CEFA che possiede una consolidata esperienza nel settore, rafforzata dalle buone pratiche sviluppate nel progetto finanziato dall’AICS ed intitolato “All Inclusive”. La nuova iniziativa – che può essere considerata come una seconda fase delle parti più significative del progetto appena menzionato - coinvolgerà il VETA, l’autorità governativa tanzaniana responsabile per la formazione professionale, e il Centro formativo Yombo specializzato nella formazione professionale di soli ragazzi con disabilità. Entrambi saranno coinvolti in interventi pilota volti a migliorare l'accessibilità al sistema di formazione professionale a Dar es Salaam e Mbeya per le persone con disabilità. Oltre a questo, l'iniziativa sosterrà attivamente la formazione degli insegnanti, fornendo loro le competenze per promuovere l'inclusione nel sistema educativo, personalizzerà i corsi di formazione professionale e prevedrà un servizio di avviamento alla carriera, specifico per ogni studente con disabilità, offrendo corsi di orientamento al lavoro e un periodo di apprendistato in azienda. The project, which aims "To end AIDS, Tuberculosis and Malaria" (General Objective), aligns with the priorities of the Tanzanian health authorities in achieving SDG 3.3. and the strategic objectives a, b and c of the Global Fund. The initiative promotes a participatory and people-centred approach aimed at improving quality of prevention, care, treatment and follow-up services in the Tanzanian context, characterized by a young population highly exposed to the risk of contracting HIV. The Shinyanga Region of Tanzania has a population of 2.241.299 people of which 35% are adolescents and young adults aged between 10 and 24 years. The prevalence of HIV in the population among adults 15-49 years is higher than national level, 5.9% (source: THIS 2016/17), with a significant gender disparity (7.2% among women while 4.3% among men). In the Shinyanga region, the % of women who have never been tested for HIV is the highest at national level, equal to 34.4% (TDHS 2022), while the percentage of women aged 15-24 with knowledge about HIV is limited to 25.7% (TDHS 2022). Since 2015, CUAMM has been operating in two districts of the Shinyanga Region, Shinyanga Municipal Council and Shinyanga District Council (total population 684,561, of which 231,397 in the 10-24 age group) and several initiatives have been implemented over the previous years in support of HIV. In particular, within the Global Fund framework, between April 2022 and September 2023, CUAMM in collaboration with the University of Florence implemented the project "Improving the health and well-being of HIV+ adolescents and young adults, AID 12349/01 /6” in the two districts mentioned above. The operational research conducted during the previous intervention highlighted the problems and challenges faced by adolescents and young people in Shinyanga, which constitutes the basis of this proposal. Specifically, in the target health centers and communities, the following problems were detected in relation to the population group aged between 10 and 24 years: Limited access to prevention and testing services especially among young people and adolescents: there is a high percentage of young people between 15 and 24 who have never been tested, especially young women and girls. Limited quality of care, treatment and follow-up services within the CTCs including i) Lack of adequate trained human resources, ii) poor differentiation of services for young people and adolescents, iii) Difficulties in ensuring adherence and follow-up -up of HIV-positive patients, v) Partial effectiveness and efficiency of the supply system Lack of an inclusive community environment and presence of discrimination and stigma towards HIV-positive people: Stigma and discrimination continue to hamper the success of HIV programs in Tanzania, with a Stigma Index of 13.7% (2020). This initiative aims to strengthen prevention and testing services at both health facilities (CTCs) and community level. Furthermore, the action will improve service delivery through the adoption of a "people-centered" approach that encourages the differentiation of services, placing more emphasis on the psychological and counseling component for people with HIV. L’iniziativa è stata sviluppata dall’ONG Medici con l’Africa CUAMM (CUAMM) in partenariato con Dipartimento di Medicina Sperimentale e Clinica, Università degli Studi di Firenze (DMSC), l’Istituto Superiore di Sanità (ISS)- Centro Nazionale per la Salute Globale (CNSG) e in stretta collaborazione con due partner operativi in loco, YAWE – Youth And Women Emancipation e NACOPHA – The National Council of People Living with HIV/AIDS. Il progetto, che ha come Obiettivo Generale quello di “Porre fine all’AIDS, alla Tubercolosi e alla Malaria”, si allinea alle priorità delle autorità sanitarie della Tanzania nel raggiungere l’SDG 3.3. e gli obiettivi strategici a, b e c del Global Fund attraverso un approccio partecipativo e incentrato sulle persone e volto al miglioramento della qualità dei servizi di prevenzione, cura, trattamento e follow-up in un contesto, quello tanzaniano, con una popolazione giovane e adolescente esposta al rischio di contrarre l’HIV. Nella Regione di Shinyanga, con una popolazione di 2.241.299 di persone di cui il 35% adolescenti e giovani adulti nella fascia tra i 10 e i 24 anni di età, la prevalenza dell’HIV nella popolazione tra gli adulti 15-49 anni è superiore al valore nazionale, 5.9% (fonte: THIS 2016/17), con una disparità di genere significativa, 7.2% tra le donne contro 4.3% negli uomini. Nella regione di Shinyanga, la % delle donne che non si è mai testata per HIV è la più alta a livello nazionale, pari al 34.4% (TDHS 2022) mentre la percentuale di donne tra i 15-24 con conoscenza su HIV è pari al 25.7% (TDHS 2022). Dal 2015, CUAMM opera in due distretti della Regione di Shinyanga, Shinyanga Municipal Council e Shinyanga District Council (pop. totale 684,561, di cui 231,397 nella fascia d’età 10-24), dove sono state realizzate nel corso degli anni precedenti diverse iniziative a contrasto dell’ HIV. In particolare, all’interno del framework Global Fund, tra Aprile 2022 e Settembre 2023, CUAMM in collaborazione con l’Università di Firenze ha implementato il progetto “Migliorare lo stato di salute e benessere degli adolescenti e giovani adulti HIV+, AID 12349/01/6” nei due distretti sopra menzionati. La ricerca operativa, parte del progetto, e la quotidiana osservazione, durante le attività, delle problematiche e delle sfide per gli adolescenti e i giovani di Shinyanga, sia sieropositivi che negativi, costituiscono le basi di questo nuovo intervento. Nello specifico, presso l’area di intervento e le unità sanitarie di riferimento, sono stati rilevati i seguenti problemi e carenze in relazione alla fascia di popolazione in età compresa tra i 10 e i 24 anni: - Scarso accesso ai servizi di prevenzione e testing, in particolare tra i giovani e adolescenti e alta percentuale di ragazzi tra i 15 e 24 anni che non si sono mai testati, soprattutto giovani donne e ragazze. - Limitata qualità dei servizi di cura e trattamento e follow-up all’interno del CTCs tra cui figurano i) Mancanza di risorse umane adeguatamente formate, ii) scarsa differenziazione dei servizi rivolti a giovani e adolescenti, iii) Difficoltà nell’aderenza e follow-up dei pazienti sieropositivi, v) Parziale efficacia ed efficienza del sistema di approvvigionamento - Mancanza di ambiente comunitario inclusivo e presenza di discriminazione e stigma verso le persone sieropositive: Lo stigma e la discriminazione continuano a rappresentare i principali ostacoli al successo dei programmi contro l’HIV in Tanzania, con uno Stigma Index pari a 13.7% (2020). L’iniziativa in oggetto vuole pertanto promuovere un rafforzamento delle attività di prevenzione e testing sia a livello di strutture sanitarie (CTCs) che nelle comunità, nonché azioni volte a promuovere un miglioramento dei servizi attraverso l’adozione di approcci “people-centered” che incentivano la differenziazione dei servizi e una maggior attenzione alla componente psicologica e di counselling per persone con HIV. The intervention area in the Longido district (Arusha region) is a high-biodiversity area located in the ecological corridor between the Amboseli and Natron ecosystems, on the border with Kenya. It is threatened by the challenges posed by climate change and demographic growth, which have increased human pressure on the land. The food security of the local communities, among the poorest in Tanzania, depends on pastoralism and is severely threatened by the loss of pasture productivity. Causes include the massive spread of invasive species that are often inedible for livestock and wildlife, loss of vegetative cover, and overgrazing, exacerbated by irregular rainfall. The main focus is environmental support, with an integrated approach that promotes gender equality, participatory development, and disaster risk reduction, in line with OECD/DAC policy priorities. Result 1 (OECD/DAC sector: 31130): Strengthening the climate change adaptation capacity of local communities by promoting improved management of land at risk of desertification. Capitalizing on Oikos' experience in the intervention area and recent research results, the project adopts a methodology aimed at reversing land degradation through rehabilitation interventions that also trigger a virtuous cycle of female empowerment. Maasai women play a central role in leading invasive species removal campaigns and pasture restoration, supported by youth groups trained by the project to constantly monitor environmental impact. Through these actions, women not only contribute to ecosystem health but also enhance their social role and economic independence. The production of fodder from healthy pastures will be the most significant tangible benefit, linking healthy pastures to communities resilient to climate change and economic shocks. Additionally, the best practices promoted by the project will be the subject of an extensive environmental education campaign in schools and villages, highlighting the role of women. These synergistic actions aim to improve pasture management, promoting a sustainable balance between people, livestock, and wildlife. Result 2 (OECD/DAC sector: 23110): Promoting climate change mitigation actions in the intervention area by strengthening international cooperation between the government agency responsible for water distribution in rural areas (RUWASA), civil society organizations, and Italian companies operating in the renewable energy sector. Specifically, Carbonsink strengthens RUWASA's capacities for accreditation to the Green Climate Fund and the creation of a specialized technical unit for managing water access projects fundable through carbon credits. On the digitalization front, a national level software will be developed by RUWASA to monitor the community-based water supply organizations (CBWSO). In the intervention area, five multi-functional photovoltaic systems managed by the local CBWSOs will be installed to improve water access for 8,409 people and provide electricity for productive activities. Simultaneously, 5,000 energy-saving stoves will be distributed to reduce deforestation and domestic emissions, with financial benefits guaranteed for 5 years by the Gold Standard certification obtained through the partner Offgridsun. Il progetto RESTORE rafforza la resilienza e la capacità di adattamento delle comunità pastorali della Tanzania ai cambiamenti climatici, migliorando le conoscenze e le capacità umane e istituzionali degli attori locali e promuovendo azioni di adattamento e mitigazione in linea con gli Obiettivi di Sviluppo Sostenibile 7, 13 e 15. L’area d’intervento nel distretto di Longido (regione di Arusha) è un'area ad alta biodiversità situata nel corridoio ecologico tra l'ecosistema Amboseli/Kilimanjaro e Natron, al confine con il Kenya. E’ minacciata dalle sfide create dai cambiamenti climatici e dall’aumento della pressione antropica sul territorio. La sicurezza alimentare delle comunità locali, tra le più povere della Tanzania, dipende dalla pastorizia ed è gravemente minacciata dalla perdita di produttività dei pascoli. Le cause includono la massiccia diffusione di specie invasive non commestibili, la perdita di copertura vegetale e il sovrapascolo, esacerbato dall’irregolarità delle piogge. Il focus principale è il sostegno all’ambiente ovvero il recupero di ambienti degradati e l’ottimizzazione dell’accesso e dell’uso delle risorse naturali (acqua e combustibili), con un approccio integrato che promuove la parità di genere, lo sviluppo partecipativo e la riduzione del rischio di disastri naturali, conformemente alle priorità politiche dell'OCSE/DAC. Il Risultato 1 (settore OCSE/DAC: 31130) rafforza la capacità di adattamento ai cambiamenti climatici delle comunità locali promuovendo la migliorata gestione dei terreni a rischio desertificazione. Capitalizzando l'esperienza di Oikos nell’area di intervento e i risultati di recenti ricerche, il progetto adotta una metodologia mirata ad invertire il processo di degrado dei terreni attraverso interventi di riqualificazione che innescano anche un meccanismo virtuoso di empowerment femminile. Le donne Maasai assumono un ruolo centrale nella guida delle campagne di rimozione delle specie invasive e nel ripristino dei pascoli, coadiuvate da gruppi di giovani formati dal progetto per monitorare costantemente la salute ambientale. Attraverso tali azioni, le donne non solo contribuiscono alla salute degli ecosistemi, ma migliorano anche il loro ruolo sociale e la propria indipendenza economica. La produzione di foraggio da pascoli sarà il beneficio tangibile più importante che lega pascoli sani e comunità resilienti ai cambiamenti climatici e agli shock economici. Inoltre, le buone pratiche promosse dal progetto saranno oggetto di una vasta campagna di educazione ambientale nelle scuole e nei villaggi, valorizzando il ruolo delle donne. Queste azioni sinergiche mirano a migliorare la gestione dei pascoli, favorendo un equilibrio sostenibile tra persone, bestiame e fauna selvatica. Il Risultato 2 (settore OCSE/DAC: 23110) promuove azioni di mitigazione dei cambiamenti climatici nell’area d’intervento rafforzando la cooperazione internazionale tra l’agenzia governativa responsabile della distribuzione d’acqua nelle aree rurali (RUWASA), le organizzazioni della società civile e le imprese italiane operanti nel settore delle energie rinnovabili. In particolare, Carbonsink rafforza le capacità di RUWASA per l’accreditamento al Green Climate Fund e per la creazione di un'unità tecnica specializzata per la gestione di progetti di accesso all’acqua finanziabili tramite crediti di carbonio. For decades Tanzania has also been suffering the effects of climate change, in particular recurring droughts and floods, which have caused devastating impacts on the agriculture, water and energy sectors. In order to meet the need of Tanzania's agrifood sector to have the capacity to adapt to climate change, the Lombardy Region has engaged excellences of the Lombardy system including universities, private business associations and public bodies, and OSC, in partnership with SAGCOT (Southern Agricultural Growth Corridor of Tanzania) and various local stakeholders to ensure the alignment of the intervention with national strategies for the agrifood sector and climate change adaptation policies, favouring synergy and complementarity with Italian cooperation initiatives. Region Lombardy proposes capacity building activities at institutional and university level for effective planning and management of climate change adaptation strategies in the agrifood sector (R1), develop participatory governance tools to strengthen the resilience of food systems and the agricultural sector in the Arusha region (R2) and Pemba Island (R3). The Arusha Region and Pemba Island are the target areas for the implementation of the project in which the Region's partners have long-term experience and good relations with local institutions. At the academic level, exchanges of expertise are planned between the University of Milan, the University of Milan Bicocca, the University of Brescia and Tanzanian universities and institutions, namely the University of Zanzibar and the Nelson Mandela African Institution of Science and Technology of Arusha. In coherence with the needs identified in the need assessment and in the dialogue with the Tanzanian institutional partner, support will be provided for the definition of climate change adaptation plans with a focus on the sustainable use of compost and the consequences of animal farming in agricultural and food systems. Under R2, stakeholders in the Arusha Region will be supported in the development of a sustainable food production model and the improvement of food governance through the Italian partners Regional Agency for Agricultural and Forestry Services (ERSAF) and the Institute for International Economic Cooperation (ICEI). With regard to the island of Pemba, R3 provides for training and awareness-raising activities for stakeholders and local communities on sustainable, regenerative and climate change resilient agriculture, integrated management of natural resources and reuse of waste and food waste in view of the circular economy to activate both a participatory spatial planning process and research and development activities at an academic level; technical support will also be provided to small local producers thanks to the presence of the Ivo de Carneri Foundation and the Seva for Africa Association. ANCI Lombardia will have the important role of fostering territorial partnerships between Lombardy municipalities and Tanzanian territorial authorities. La Tanzania da decenni soffre le conseguenze del cambiamento climatico, in particolare di siccità e inondazioni ricorrenti, che hanno causato impatti devastanti sui settori dell'agricoltura, dell'acqua e dell'energia. A seguito di una richiesta di supporto da parte del governo della Tanzania per sviluppare la capacità di adattamento ai cambiamenti climatici del settore agroalimentare, Regione Lombardia ha coinvolto eccellenze del sistema lombardo tra università, associazioni di imprese private ed enti pubblici, e OSC, in raccordo con SAGCOT (Southern Agricultural Growth Corridor of Tanzania) partner tecnico locale indicato dal governo nazionale, e con altri stakeholder locali e Università a Pemba e ad Arusha. Sarà garantito l’allineamento dell’intervento alle strategie nazionali per il settore agroalimentare e alle esistenti politiche di adattamento al cambiamento climatico della Tanzania, favorendo sinergia e complementarità con altre iniziative della cooperazione italiana e europea. L’intervento si articola in attività di capacity building a livello istituzionale e universitario per una efficace pianificazione e gestione delle strategie di adattamento al cambiamento climatico nel settore agroalimentare (R1), sviluppare strumenti di governance partecipata per rafforzare la resilienza dei sistemi alimentari e del settore agricolo nella regione di Arusha (R2) e nell’isola di Pemba (R3). La Regione di Arusha e l’Isola di Pemba sono le località target di realizzazione di progetto in cui i partner della Regione hanno esperienze di lungo periodo e buone relazioni con le istituzioni locali. A livello accademico sono previsti scambi di competenze tra l’Università degli Studi di Milano, l’Università di Milano Bicocca, l’Università di Brescia e università e istituzioni tanzaniane, quali l’Università degli Studi di Zanzibar e Il Nelson Mandela African Institution of Science and Technology di Arusha. Coerentemente con i bisogni individuati nel need assesment e nel dialogo con il partner istituzionale tanzaniano verrà fornito un supporto alla definizione e implementazione di piani di adattamento ai cambiamenti climatici con un focus sull’utilizzo sostenibile del compost e sulle conseguenze dell’allevamento nei sistemi agricoli e alimentari. Nell’ambito del R2 verranno supportati gli stakeholder nella Regione di Arusha per lo sviluppo di un modello di produzione alimentare sostenibile e il miglioramento della governance alimentare grazie a partner italiani quali l’Ente Regionale per i Servizi all'Agricoltura e alle Foreste (ERSAF) e l’Istituto Cooperazione Economica Internazionale (ICEI). Per quanto riguarda l’isola di Pemba l’R3 prevede attività di formazione e sensibilizzazione per stakeholder e comunità locali su agricoltura sostenibile, rigenerativa e resiliente al cambiamento climatico, gestione integrata delle risorse naturali e riuso degli scarti e food waste in ottica di economia circolare per attivare sia un percorso di pianificazione territoriale partecipativa che attività di ricerca e sviluppo a livello accademico; inoltre verrà fornito supporto tecnico ai piccoli produttori locali grazie alla presenza di Fondazione Ivo de Carneri e l’associazione Seva for Africa. Italian Agency for Cooperation and Development Nourishing the Future is a multsectoral initiative tackling malnutrition that combines interventions in agriculture, health and nutrition, with the aim of improving the health and nutritional status of communities Nutrendo il futuro è un'iniziativa integrata di lotta alla malnutrizione che combina interventi in ambito agricolo, sanitario e nutrizionale, con l'obiettivo di migliorare lo stato di salute e nutrizionale delle comunità residenti nelle regioni di Iringa e Njombe (Tanzania), in particolare quello di mamme e bambini U5. Si propone di intervenire in 60 villaggi situati nei Distretti di Iringa, Kilolo, Mufindi e Njombe, dove la malnutrizione cronica tra i bambini U5 raggiunge tassi d'incidenza tra i più alti del Paese. I partner hanno identificato come principali determinati dello stato di salute e nutrizionale dei beneficiari: i) la scarsa diversificazione della dieta di mamme e bambini U5 e ii) la limitata copertura e scarsa qualità dei servizi sanitari e nutrizionali. In risposta a queste problematiche si propone un intervento integrato che si sviluppa intorno a 4 componenti: 1. Agricola: Al fine di vedere aumentata disponibilità, varietà ed accessibilità degli alimenti a disposizione delle comunità target (R1.1) si propone di intervenire su produttività agricola, riduzione delle perdite post-raccolto, diversificazione delle produzioni familiari e incremento della redditività delle attività agricole. 2. Buone pratiche: Per favorire l'adozione di buone pratiche igienico-sanitarie, nutrizionali e di nurturing care, specialmente tra mamme e caregiver (R1.2) si propongono interventi di sensibilizzazione comunitaria di tipo partecipativo, il rafforzamento delle attività di screening dei soggetti più a rischio da parte dei Community Health Workers e il supporto ad attività ECD nei villaggi. 3. Sanitaria: Per migliorare copertura e qualità dei servizi sanitari di salute materno-infantile e nutrizionali (R2.1) e ridurre nei distretti target stunting e morbilità tra bambini U5 e mamme, si prevedono interventi di supervisione e formazione del personale sanitario; la fornitura di materiali, equipaggiamento e supplementi nutrizionali alle unità sanitarie e l'introduzione al loro interno di servizi ECD. 4. The proposal is framed within SDG 8 - Agenda 2030 ‘Sustained, inclusive and sustainable economic growth, full and productive employment and decent work for all’. The project proposal, through targeted training and capacity building actions, intends to transfer sustainable agricultural practices to coffee operators (OCSE DAC 11330 Vocational Training) contributing to the proper management of land and natural resources (PO OCSE Aid to environment), protecting biodiversity (RIO MARKER). The focus on training on the rational and efficient management of water resources in coffee processing that aims to reduce pollution caused by wastewater (OCSE DAC 14050 Waste management), and on climate change adaptation strategies (RIO MARKER), contribute to strengthening the resilience of smallholders and agricultural cooperatives (OCSE DAC 31194 Agricultural Cooperatives). Income diversification, technological upgrading and innovation are essential components aimed at strengthening the professional skills of young people and women (OCSE DAC 31181 Agricultural Education) engaged in the agricultural sector by giving them the tools to encourage sustainable and profitable quality agricultural production by enhancing local human and environmental resources. Achieving higher levels of economic productivity through microcredit actions for coffee crop production and improving product quality contributes to increasing the level of food security of households in the three intervention regions (PO OCSE Nutrition). The project will adopt a gender mainstreaming approach in all its phases with the objective of fostering a renewed sense of empowerment and economic independence through trainings aimed at strengthening their leadership within the family production realities (PO OCSE Gender Equality). Entrepreneurship training for young people and women and equipment for on-site cooperatives will boost the development of the productive environment and thus the creation of further employment opportunities, exchanges and events to stimulate business opportunities between Italian and Tanzanian companies (PO Trade development). The target groups are coffee operators, in particular: small producers, agricultural cooperatives to which they belong, government officials, farmer leaders and environmental activists. The involvement of these actors will foster the development of new synergies and collaborations that will become a tool for social cohesion and ensure sustainability of the initiative, thus promoting good governance practices and fostering the link between the Italian partners and the local partnership for a sustainable cooperation in the development of the coffee supply chain in the three regions (PO OCSE good governance). The partnership moves in a synergic and multidisciplinary manner. On the one hand institutional bodies (Autonomous Region of Friuli Venezia Giulia and the local government authorities), on the other hand CIHEAM Bari (International Org.), important scientific bodies (University of Trieste), private companies (illycaffè), foundations (Ernesto Illy Foundation) and local bodies (Cafè Africa Tanzania and Tanzania Coffee Board) working synergically in the promotion of support policies and the production of sustainable quality coffee. La proposta “Verso la nuova generazione di produttori di caffè di qualità sostenibile nel Sud della Tanzania” si è sviluppata a partire da un’analisi dei bisogni e dei problemi della filiera del caffè nelle tre regioni di Mbeya, Songwe e Ruvuma in Tanzania con l’obiettivo di favorire la crescita occupazionale di giovani e donne tanzaniani per lo sviluppo e l’innovazione di una filiera sostenibile del caffè. L’attenzione posta alla formazione sulla gestione razionale ed efficiente delle risorse idriche nella lavorazione del caffè che mira a ridurre l’inquinamento causato dalle acque reflue (OCSE DAC 14050 Waste management) e, sulle strategie di adattamento ai cambiamenti climatici (RIO MARKER), contribuiscono a rafforzare la resilienza dei piccoli produttori e delle cooperative agricole (OCSE DAC 31194 Agricultural Cooperatives). La diversificazione del reddito, l’aggiornamento tecnologico e l’innovazione sono componenti essenziali volte a rafforzare le competenze professionali di giovani e donne (OCSE DAC 31181 Agricultural Education) impegnate nel settore agricolo dando loro gli strumenti per incoraggiare una produzione agricola di qualità sostenibile e redditizia valorizzando le risorse umane e ambientali locali. Il raggiungimento di livelli più elevati di produttività economica grazie ad azioni di microcredito per la produzione di colture consociate al caffè e il miglioramento della qualità del prodotto contribuisce ad aumentare il livello di sicurezza alimentare delle famiglie delle tre regioni di intervento (PO OCSE Nutrition). Il progetto adotterà un approccio di gender mainstreaming in tutte le sue fasi con l’obiettivo di favorire un rinnovato senso di empowerment e indipendenza economica, grazie a formazioni mirate a rafforzare la loro leadership all’interno delle realtà produttive familiari (PO OCSE Gender Equality). L’affiancamento all’imprenditorialità per giovani e donne e gli equipaggiamenti per le cooperative in loco daranno impulso allo sviluppo del tessuto produttivo e quindi alla creazione di ulteriori opportunità di impiego, scambi ed eventi per stimolare opportunità di business tra aziende italiane e tanzane (PO Trade development). I gruppi target sono gli operatori caffeicoli, in particolare: piccoli produttori, cooperative agricole a cui afferiscono, funzionari governativi, agricoltori leader e attivisti ambientali. Il coinvolgimento di tali attori favorirà lo sviluppo di nuove sinergie e collaborazioni che diventeranno strumento di coesione sociale e assicureranno sostenibilità all’iniziativa, promuovendo così pratiche di good governance e incentivando il legame tra i partner italiani e il partenariato locale per una compartecipazione sostenibile allo sviluppo della filiera del caffè nelle tre regioni (PO OCSE good governance). In doing so, the project adopts a multi-sector strategies aiming at: a) obtaining the registration of an official training curriculum on domestic work from the Vocational and Education Training Authority of Tanzania. This curriculum has been tested by CVM in the past and has shown efficacy in improving the self-awareness of the trainees and improving their working conditions. The registration of the curriculum is paralleled by the training of teachers in public vocational training centers in the target regions to ensure its uptake. b) encourage the participation of Domestic Workers in Trade Unionsthrough the establishment of self-help groups linked with the national trade union CHODAWU in the target regions. These groups will receive specific training courses on their rights as workers and women. Furthermore, a rotative credit fund for training will be made available to allow DWs facing difficult economic conditions to access professional training courses. c) Increase the social visibility of DWs by means of communication events - also at political and academic level - that can push forward the agenda of the recognition of DWs’ rights. Il progetto emPoWeReD si prefigge come scopo il miglioramento del livello di formazione professionale della categoria delle Lavoratrici Domestiche in Tanzania, per incrementare la loro capacità di rivendicare i propri diritti e uscire dall’informalità lavorativa. Nel perseguire questo scopo il progetto adotta una strategia multisettoriale che mira a: a. ottenere la registrazione di un curriculum formativo ufficiale sul lavoro domestico presso l’Autorità Tanzana per la Formazione Professionale. The project’s impact is to improve the social and public health condition and the resilience, including climate changes and hazards, of the population in the target regions. In Tanzania, only 64% of households have access to at least basic drinking water service , 55% to basic sanitation , and 48% of the population to basic hygiene practices . In Manyara and Dodoma regions, access to water is still below the national average (50% and 63% respectively) while access to basic hygiene services stands at 46.1% and 59.5% respectively . The intervention is aligned to the strategic priorities outlined in the nation’s “Five-Year Medium Term Strategic Plan 2019/20-2023/24”, Water Sector Development Programme Phase Three (WSDP III) 2022/23 – 2025/26 and the National Water Policy 2002 (Version 2025) of Ministry of Water, which recognize the importance to enhance the provision and sustainability of water and sanitation services and to foster human capital in the WASH sector. The intervention logic is structured towards three main pillars. The first pillar aims at improving the provision of safe and resilient water supply in the rural areas, using innovative technologies (Result 1). The second pillar of the strategy will focus on strengthening the institutional capacity (Rural Water Supply and Sanitation Agency, the Community Based Water Supply Organizations and local authorities) to manage water and sanitation services effectively, financially sustainably, and in a participatory manner. (Result 2). The last pillar of the intervention logic will focus on ensuring the availability of improved hygiene and sanitation services and the promotion of hygiene in schools, healthcare facilities, and public community areas (Result 3). The project will be implemented by an Italian Civil Society Organizations (CSOs) selected through a competitive call for proposal and in partnership with local partners and from “Italian System”. An inclusive and gender-sensitive approach will ensure that girls, women, persons with disabilities, and the elderly have access to safe and affordable drinking water and sanitation services, using schools and health facilities as entry points to reach the most vulnerable. Le metodologie e i risultati conseguiti verranno promossi dalla Regione attraverso riunioni di vario livello. R2. I COWSO operano in modo efficiente nella gestione dei servizi idrici e igienico-ambientali di qualità e inclusivi. I COWSO sono limitati nella loro efficienza dalla mancanza di supporto istituzionale e da inadeguate capacità tecniche e gestionali. Si prevede la registrazione di 40 nuovi COWSO e attività di rafforzamento per i 46 esistenti, mobilitando le comunità e coinvolgendo le donne con il supporto del partner UCRT. L'ingresso dei privati verrà facilitato con il WASH Service Center nel Simanjiro e sostenendo la creazione di un Prestatore di Servizi nel Kiteto. R3. Aumentate le infrastrutture di approvvigionamento idrico attraverso sistemi ad alta efficienza energetica (pompe solari), processi di trattamento delle acque sostenibili. KISIMA riabiliterà 6 impianti idrici in villaggi selezionati con Regione e Distretti, sostituirà 3 impianti diesel con sistemi solari e installerà un sistema di desalinizzazione, progettato dal partner italiano Idrodepurazione srl per l'ospedale di Kibaya. R4. Le condizioni igienico-ambientali nelle scuole primarie, nei dispensari e negli spazi pubblici migliorano. Il progetto costruirà 59 latrine VIP con accesso ai disabili. Le campagne di sensibilizzazione realizzate in partnership con MAMADO favoriranno la riduzione delle malattie e una maggior presenza delle bambine nelle scuole. L'impatto di KISIMA si avrà a più livelli. Più di 240.000 abitanti (51 donne) dei 2 Distretti avrà accesso all'acqua pulita e a servizi igienicoambientali e la riduzione dei costi di funzionamento dei sistemi con la adeguata gestione finanziaria dei COWSO ridurranno la spesa delle famiglie. Il miglioramento dei servizi igienicoambientali nelle scuole inoltre genererà un impatto sulla scolarizzazione, in particolare delle bambine e dei gruppi svantaggiati. A livello istituzionale a fine progetto Regione e Distretti avranno le competenze per gestire le risorse idriche in modo sostenibile, generando impatto sulla resilienza della popolazione agli shock provocati dal cambiamento climatico (siccità). Il rafforzamento istituzionale creerà un clima di fiducia e affidabilità tra istituzioni e utenti con impatto nella sostenibilità futura del servizio. Fundamental pillars of the project are therefore: 1) the strengthening of basic health facilities in terms of infrastructure, equipment and training of social and health personnel; 2) the enhancement of the figure of community health workers (CHWs) through the conduct of specific courses and material support for the conduct of activities (purchase of mobile phones/ tablets and bicycles). The project will also cross-cuttingly include the implementation of activities to foster synergy between TB/HIV programs and operational research activities with a specific focus on paediatric and adolescent age (0-14). The direct beneficiaries of the project will therefore be basic health facilities, social and health workers and CHWs and the 115.050 inhabitants of the 26 selected villages. The planned activities will take place within 24 months. The strengthening of healthcare facilities includes the purchase of 15 standard microscopes and other diagnostic supplies to improve the diagnosis of tuberculosis and the identification of pharmacological resistance to treatment (MDR), and the purchase of a Gene Expert device and related supplies. Detection rate still seems to be an area where considerable improvement is possible given the gap between the cases actually diagnosed and the estimates of cases present in the territory. Another strength will be the training and involvement of 104 CHWs, who will be encouraged to actively participate in the promotion of initiatives for the identification and treatment of TB cases, to promote the therapeutic adherence of positive TB patients in following the correct intake of therapy, to facilitate the integration of national TB/HIV programs in their respective villages and to provide adequate information on the rights of the population to access health facilities. The overall aim of the project is to reduce the burden of diseases for TB and HIV in target populations by broadening the scope of local control programmes. Partendo da queste carenze, già incontrate nella realizzazione di programmi per la diagnosi HIV e delle malattie correlate nella Regione di Dodoma e in linea con i progetti attivi promossi dal TNCM tanzaniano, il presente progetto si concentrerà nel distretto di Kondoa (Regione di Dodoma), in 26 villaggi selezionati particolarmente isolati, con l’obiettivo di rinforzare il sistema sanitario locale e migliorare l’accessibilità e la qualità delle cure per TB e HIV. Scopo complessivo del progetto è ridurre il “burden of diseases” per la TB e HIV nelle popolazioni target, ampliando la portata dei programmi di controllo a livello locale. Per questo nel rafforzamento delle strutture sanitarie è incluso l’istallazione di pannelli solari, l’acquisto di 15 microscopi standard con altre forniture diagnostiche per migliorare la diagnosi della tubercolosi e l’identificazione delle resistenze farmacologiche al trattamento (MDR) e l’acquisto di un apparecchio GeneXpert e relativo materiale di consumo. L’identificazione dei casi (detection rate) è un ambito in cui è possibile un considerevole miglioramento vista la forbice esistente tra i casi realmente diagnosticati e le stime dei casi presenti nel territorio. Altro punto di forza sarà la formazione e il coinvolgimento di 104 CHWs, che verranno incoraggiati a partecipare attivamente alla promozione di iniziative per l’identificazione e la cura dei casi di TB, a favorire l’aderenza terapeutica dei pazienti TB positivi nel seguire la corretta assunzione della terapia, a favorire l’integrazione dei programmi nazionali TB/HIV nei rispettivi villaggi e a fornire informazioni adeguate sui diritti della popolazione per accedere alle strutture sanitarie. In parallel, it also intends to improve the quality of HIV-related health services offered at the district level by capacity development about prevention and control of HIV among health staff working in three facilities and Community Health Workers active in the respective catchment area of Shinyanga Regional Referral Hospital (SRRH), Bugisi HC and Ngokolo HC. Finally, the project will enhance knowledge on barriers to accessing services and behavioural knowledge to plan for future ad hoc interventions /programme addressing needs and constraints taking into account also the impact of COVID-19. Il progetto, della durata di 18 mesi, mira quindi, attraverso attività di ricerca operativa, ad aumentare l’accesso e a migliorare la qualità dei servizi di prevenzione, diagnosi precoce e trattamento/follow up dell’HIV in Tanzania (Obiettivo Specifico), producendo conoscenza basata su evidenze scientifiche circa i fattori che favoriscono o impediscono l’accesso ai servizi e l’impatto indiretto della pandemia di COVID-19 sulla salute, inclusa quella mentale, di adolescenti e giovani HIV+. Tali ricerche, svolte in coordinamento con le autorità sanitarie e di ricerca locali (NIMR) saranno rilevanti alla formulazione di politiche sanitarie sia a livello locale che provinciale e nazionale, volte al rafforzamento della resilienza dei servizi a shock esterni come quello attuale. A complemento di tali attività, il progetto intende altresì migliorare la qualità dei servizi sanitari per l’HIV offerti presso le aree di intervento attraverso il rafforzamento delle competenze del personale sanitario in servizio nei CTCs e degli attivisti comunitari in merito a misure di prevenzione e contenimento di HIV e COVID-19, in modo tale da combattere altresì lo stigma dovuto al timore di potersi infettare. Tali attività formative verranno pianificate e svolte in partenariato con il Regional Health Management Team- autorità sanitaria responsabile a livello regionale - in modo tale da assicurare l’ownership e l’allineamento con le linee guida locali. Infine, il progetto cercherà di far fronte all’accresciuto malessere psicologico dovuto alle limitazioni imposte dalla pandemia di COVID-19 attraverso la realizzazione di sessioni di counselling psicologico nell’ambito della ricerca qualitativa. Per fare ciò, il progetto sosterrà i partner locali nella promozione di servizi sanitari preventivi e curativi di qualità ed efficaci, eliminando le barriere di accesso per le popolazioni più vulnerabili colpite dalle tre malattie (l'"ultimo miglio"), incoraggiando misure a livello di comunità per aumentare l'accessibilità ai servizi e garantendo la salvaguardia dei diritti dei pazienti (Obiettivo Specifico). Al fine di raggiungere l’obiettivo specifico dell’iniziativa, l’intervento del consorzio durante il progetto consentirà Il Miglioramento dei servizi di prevenzione delle strutture sanitarie a livello distrettuale attraverso interventi basati su CRG (R1), l’aumento dell'infrastruttura dei servizi sanitari comunitari per la diagnosi e il trattamento di HIV, TBC e malaria (R2), il consolidamento di un sistema di medicina di prossimità per la prevenzione e il trattamento di HIV, TBC e malaria (R3). L'approccio metodologico principale della proposta sarà quello comunitario, dei diritti e di genere. Infatti, come suggerito dalla "National Community, Rights and gender implementation guide for TB responses 2020", questo tipo di approccio garantirà l'efficacia della proposta, concentrandosi sui bisogni e sulla partecipazione attiva dei beneficiari più vulnerabili e, quindi, garantendo la sostenibilità a lungo termine dell'intervento. KISIMA is articulated into 4 results: R1. Region and Districts provide an L'intervento si propone di garantire e aumentare l'accesso equo ed economicamente sostenibile ai sistemi idrici e ai servizi igienico-ambientali nei due distretti. KISIMA si articola in 4 risultati: R1. Le istituzioni regionali e distrettuali forniscono un servizio efficace, coordinato e competente per garantire un'offerta equa, accessibile e migliorata dei servizi idrici e igienico-ambientali. Il rafforzamento delle istituzioni verrà raggiunto attraverso formazione e fornitura di strumenti che rafforzino la conoscenza e le competenze dei funzionari e dei tecnici. Ciò comprende: un'analisi idrogeologica del territorio con il partner Università della Bicocca, un database regionale per archivio e uso dei dati, un supporto al Laboratorio di analisi di Orkesumet. Le metodologie e i risultati conseguiti verranno promossi dalla Regione attraverso riunioni di vario livello. R2. I COWSO operano in modo efficiente nella gestione dei servizi idrici e igienico-ambientali di qualità e inclusivi. I COWSO sono limitati nella loro efficienza dalla mancanza di supporto istituzionale e da inadeguate capacità tecniche e gestionali. Si prevede la registrazione di 40 nuovi COWSO e attività di rafforzamento per i 46 esistenti, mobilitando le comunità e coinvolgendo le donne con il supporto del partner UCRT. L'ingresso dei privati verrà facilitato con il WASH Service Center nel Simanjiro e sostenendo la creazione di un Prestatore di Servizi nel Kiteto. R3. Aumentate le infrastrutture di approvvigionamento idrico attraverso sistemi ad alta efficienza energetica (pompe solari), processi di trattamento delle acque sostenibili. KISIMA riabiliterà 6 impianti idrici in villaggi selezionati con Regione e Distretti, sostituirà 3 impianti diesel con sistemi solari e installerà un sistema di desalinizzazione, progettato dal partner italiano Idrodepurazione srl per l'ospedale di Kibaya. R4. Le condizioni igienico-ambientali nelle scuole primarie, nei dispensari e negli spazi pubblici migliorano. Il progetto costruirà 59 latrine VIP con accesso ai disabili. Le campagne di sensibilizzazione realizzate in partnership con MAMADO favoriranno la riduzione delle malattie e una maggior presenza delle bambine nelle scuole. L'impatto di KISIMA si avrà a più livelli. Più di 240.000 abitanti (51% donne) dei 2 Distretti avrà accesso all'acqua pulita e a servizi igienicoambientali e la riduzione dei costi di funzionamento dei sistemi con la adeguata gestione finanziaria dei COWSO ridurranno la spesa delle famiglie. Il miglioramento dei servizi igienicoambientali nelle scuole inoltre genererà un impatto sulla scolarizzazione, in particolare delle bambine e dei gruppi svantaggiati. A livello istituzionale a fine progetto Regione e Distretti avranno le competenze per gestire le risorse idriche in modo sostenibile, generando impatto sulla resilienza della popolazione agli shock provocati dal cambiamento climatico (siccità). Il rafforzamento istituzionale creerà un clima di fiducia e affidabilità tra istituzioni e utenti con impatto nella sostenibilità futura del servizio.
