South Sudan is experiencing multiple crises including these unprecedented levels of floods, an influx of returnees and refugees because of the conflict in Sudan, severe food insecurity and hyperinflation. The rainy season has become increasingly severe due to climate change, stretching communities’ ability to cope with the devastation, and in some cases causing permanent displacement of communities. This action prioritises the five Payams in Fangak County by providing Cash for Food to the most vulnerable households in line with cluster recommendations in Manajang, Pulita and Barboui payams and provide Shelter NFIs to households in need of shelter in Pulita, Mareang and Paguir Payams. As part of this emergency declaration, a joint InterAgency Flash Appeal developed by UNHCR in coordination with the Government and Partners was released, with eight partners appealing for a combined US$33.2 million to respond to 90,000 new arrivals over a four-month period. UNHCR has launched a four-month emergency appeal for the period December 2025 through March 2026. Under this appeal UNHCR is requesting US$47.2 million to support the immediate and ongoing response to IDPs in eastern DRC, reinforce preparedness, and sustain life-saving protection and assistance for refugees and returnees arriving in Burundi as well the broader region. Priority sectors: WASH, Shelter/NFIs, Protection and multipurpose cash assistance (MPC) as a modality to meet the most urgent needs of displaced families. Beneficiaries will be targeted with a combination of activities from different sectors. On the DRC side of the border, the response will focus on Uvira terriory, with the following health areas being targeted by partners: Kalundu State Health Area, Kalundu CEPAC Health Area, and Kalundu Catholic Health Area, Kiliba CEPAC, Kasenga and Kagando Health Areas, Hongero, Kilibas Ondes, Runingu and Ndunda. In Burundi, the response will focus on the reception sites close to the border, such as Cishemere and Kansenga camps. Where possible, JR partners will coordinate with the aim of complementarity and/or targeting the same beneficiaries, for example in Cishemere camp in Burundi. Importantly, the response will continue to monitor the flow of refugees. It is to be expected that the Burundian authorities will move refugees to more permanent camps in the east of the country. WSH-R1 activity 2: Distribution and installation of hand-washing equipment and soap in 4 schools and 1 health facility (only soap for handwashing) During this quarter, the project distributed the soaps in the Nemba and Buzimba primary schools, and in the Sebele and Nemba Child-Friendly Spaces, in order to reinforce hygiene practices such as handwashing and thus contribute to hygiene promotion and the prevention of water-borne diseases in school environments. A total of 4,228 beneficiaries used the soap purchased by the project in the Nemba and Sebele primary schools, as well as in the child-friendly spaces and health centres in Nemba and Sebele. In Sebele: 2,787 beneficiaries (1,030 girls and 1,215 boys under the age of 18 at school and at the CFS, as well as 430 women and 112 men over the age of 18 at the health centre) and in Nemba: 1,501 beneficiaries (554 girls and 655 boys under the age of 18 at school and at the CFS, as well as 232 women and 60 men over the age of 18 at the health centre). WSH-R1 activity 3: Facilitate and monitor implementation of 4 school brigade action plans, ADED continues to regularly monitor the implementation of the action plans drawn up by the school brigades that received support during the last year. This approach is designed to assess the progress of planned activities, identify the need for further support and reinforce the sustainability of student-led initiatives to promote hygiene, environmental protection and responsible citizenship in the school environment. The soap and handwashing kits distributed by the project during the first quarter continued to be used by 4,507 beneficiaries in Fizi, including 595 at the Nemba and Buzimba primary schools (252 girls under the age of 18, 329 boys under the age of 18, 1 female teacher over the age of 18 and 13 male teachers over the age of 18), 719 in the child-friendly spaces in Sebele and Nemba (396 girls under the age of 18 and 323 boys under the age of 18) and 3,193 beneficiaries at the health centres in Nemba and Sebele (1,597 girls under the age of 18, 1,683 boys under the age of 18, 672 women over the age of 18 and 555 men over the age of 18). This represents a total of 4,507 beneficiaries reached, reflecting the positive results of the hygiene awareness campaigns organized by the project. At the period of reporting, the project had already reached a total of 11,029 beneficiaries (7,759 in Fizi, and 3,270 in Masisi) under this indicator. During this quarter, the project distributed handwashing kits, soap and hygiene materials to the Bigiri and Buguli primary schools supported in the Masisi area. The following materials were distributed to the two schools: 32 handwashing stations, 32 bucket taps, 32 small basins, 10 boxes of soap, 28 buckets, 20 hard cleaning brushes, 4 spades, 32 local brooms, 32 squeegees, 20 pairs of plastic gloves, 20 empty containers for drinking water, 320 cups, and 20 blanket cloths. This activity reached a total of 2,434 new beneficiairies, including 1,023 girls, 1,266 boys, 18 women, 34 men, and 41 girls and 52 boys living with disabilities. In Fizi, this same activity reached 7,672 beneficiaries, including 2,376 girls, 2,220 boys, 1,627 women, 1,408 men, as well as 11 girls, 11 boys, 8 women and 11 men living with disabilities. The designs of the works are available Fizi Health Zone: The schools were selected jointly by the local partners and the education inspectorate. 2 schools have already been identified, a technical assessment needs have been made, an infrastructure design has already been shared by Help a Child, and the tender document is being finalized to select suppliers of building materials and construction service providers. As a result of awareness-raising activities carried out in the Fizi area during this quarter as part of the CLTS approach, 701 households have built their own hygienic latrines in the villages of Nemba and Sebele. These latrines are used daily by 4,559 members of these households, including 1,743 from Nemba and 2,816 from Sebele. Public latrines will also be built in the area with the support of the project over the next quarter to facilitate access to hygienic toilets for the local population. The indicator figure for this quarter (578 beneficiaries) represents the number of students who have been using the latrines built by the project since 2024. This includes 335 beneficiaries at Nemba Primary School (168 girls under the age of 18, 160 boys under the age of 18, and 7 male teachers over the age of 18) and 243 beneficiaries at Buzimba Primary School (83 girls under the age of 18, 153 boys under the age of 18, 1 female teacher over the age of 18 and 7 male teachers over the age of 18). Physico-chemical and bacteriological analysis carried out. The quantitative and estimated quotes are currently being developed. 3 water point management committees were revitalized and trained by the Health Zone. A total of 28 people were trained on the roles and management of water points. Each committee is made up of 7 people and a village chief. In this training, 2 health committee presidents and 2 full-time nurses were involved to support these members. Fizi Health Zone: A joint technical assessment between ADED, Help a Child, the Fizi health zone and affected communities was organized in the health areas of Nemba and Sebele. The assessment revealed that the need for a quality water supply remains enormous in these 2 communities. It is worth mentioning the particularity of Nemba, where the women spoke out about the dangers they face when searching for water in the Nemba river. In fact, this river contains several crocodiles that capture and kill the population, especially the women who go there to wash dishes, use the toilet and look for water for domestic use. Concordant sources in the area report that 17 people have been killed by crocodiles since 2019, amoung them, 10 women and 7 men. This situation is also exacerbated by the presence of hippos, which destroy the fields around the Nemba river. Given that this river is the only source of water supply, people continue to use it, but at a low rate, which poses a hygiene hazard, as they do not have access to clean water in sufficient quantities and on time. This being the case, the people of Nemba have expressed the wish to see a water supply system in the form of a mini-adduction system with standpipes in the village, instead of mechanic water springs and gravity water sources, to ensure the protection of women. Nemba has been a landlocked area for over three decades and was once a stronghold for armed groups. The beneficiaries reached in Fizi and Masisi in 2024 continue to use the 7 water points set up by the project last year. The project also continues to support the water committees set up last year. Work to set up other water sources in Fizi and Masisi will begin in the next quarter. At the period of reporting, the project had already reached a total of 13,354 beneficiaries (7,740 in Fizi and 5,614 in Masisi) under this indicator. During this quarter, 21,167 beneficiaries in the Fizi region (6,774 girls under the age of 18, 6,350 boys under the age of 18, 4,445 women over the age of 18 and 3,598 men over the age of 18) had access to drinking water thanks to the structures built by the project in 2024, which continue to serve six villages, including two in the Sebele health zone (Sebele 1 and Seble 3) and four villages in the Nemba health zone (Nemba 1, 2, 3 and 4). For coordination: The DRA JR 2024-2026 project was officially presented to 80 people representing the Masisi community jointly by Help a Child, War Child and KUA. This activity made it possible to inform the beneficiaries, key stakeholders and the main local/national and international partners about this 3-year project, by encouraging their involvement in the implementation for the achievement of results in the different sectors of the intervention. For contractualisation : A collaboration agreement was signed with Holland Green Techm field office in Burundi to provide technical support to ensure that agricultural activities are adaptive to the climate and integrate modern farming practices. Starting activitiés Furthermore, given the changing context of the Masisi health zone, a mission to update humanitarian needs was carried out in February 2024. HCC succeeded in formalizing collaboration protocols with state services such as the Territorial Inspectorate of Agriculture, the Central Office of the Fizi Health Zone, the Division of Social Affairs (DIVAS), the Territorial Inspectorate of 'Breeding. The DRAJR partners working in this area also jointly presented the project at the community level and targeted the schools which will benefit from the project activities, notably E.P:Nemba. A meeting with the management of E.P Nemba and the Parents' Committee (COPA) was organized to explain the merits of the School Garden approach. HCC also conducted a technical assessment on Nemba to have a general overview of the Zone in terms of the conduct of food security activities. The call for tenders for the supply of agricultural inputs has been published and the supplier selected. The call for tenders for the supply of goats is in progress. South Kivu (Fizi Health Zone). FSC-R1 activity 1: Distribution of seeds and tools (vegetable crops, food crops, tools) for 400HHs Given the delay caused by the worsening security situation in the area and, above all, the loss of the B agricultural season, it was decided to recover the season by promoting market garden crops. An order for the delivery of seeds has been placed with the supplier. Over the next quarter, each household will receive a seed kit comprising 40g onion, 30g cabbage, 30g tomato, 30g eggplant, 50g amaranth and 10g watermelon. FSC-R1 activity 2: Technical support of people for the planting and monitoring of their vegetable gardens (400 households). Under the guidance of agronomist monitors, project participants are supported in agricultural activities in community fields. During this quarter, preparatory work in the fields (mowing, ploughing, planting beds) enabled 389 beneficiary households to begin installing seedbeds for the promoted crops. These 389 households comprise 3,146 project beneficiaries who are registered in the project database. FSC-R1 activity 3: Prevention of malnutrition: awareness and promotion of good nutritional practices in Health center, Communities and school Through a collaboration agreement between HCC and the Fizi Health Zone office, awareness-raising campaigns on infant and young-child feeding are frequently carried out during CPS, CPN and CPON activities at the health center and community level. The team of 10 RECOs (Relais Communautaires) relay key messages on the fight against malnutrition. During the first quarter, 652 pregnant and breastfeeding women were reached by awareness-raising messages. The sub-themes developed were the window of opportunity, optimal nutrition, 4-star nutrition, essential good nutrition practices, complementary nutrition, etc. 5 cooking demonstrations (involving 291 pregnant and breastfeeding women) and 188 home visits by 188 pregnant and breastfeeding women over the age of 18 were also organized. FSC-R1 activity 4: Set up 2 school gardens benefitting 600 students Jointly, the partners working in Fizi organized a mission to target schools in 4 health areas (Nemba, Sebele, Malinde and Simbi) from March 13 to 18, 2025, under the guidance of the EPST supervisory authority. The last activity to be carried out is the technical evaluation to select the schools that meet the pre-established criteria. Over the next quarter, the bleachers will be installed in the schools identified. FSC-R1 activity 10: Raising awareness of Environnemental education and climate change in schools and communities Awareness-raising campaigns on climate change and environmental protection are carried out in the community and in schools (EP Buzimba and EP Nemba). 579 pupils were reached at school level, including 249 girls and 330 boys. In collaboration with local leaders, sites at risk of erosion have been identified At the time of reporting, the project had already reached 5,891 beneficiaries, including 3,629 in Fizi and 2,262 in Masisi. South Kivu (Fizi Health Zone). FSC-R1 activity 1: Distribution of seeds and tools (vegetable crops, food crops, tools) for 400HHs During this quarter, the beneficiaries' agricultural activities were disrupted by insecurity and climatic disturbances during the B agricultural season. To support the beneficiaries, the project distributed an agricultural input kit to each of the 387 beneficiary households in the area, consisting of 40g of onions, 30g of cabbage, 30g of tomatoes, 30g of aubergines, 100g of amaranth and 1 hoe to enable them to grow market garden crops. Each of the 20 SHGs in the area also received 2kg of amaranth seeds to sow in their community fields. FSC-R1 activity 2: Technical support of people for the planting and monitoring of their vegetable gardens (400 households). The 389 active households continue to benefit from local technical support from agricultural instructors, from preparatory activities (opening fields, clearing land, ploughing) to setting up seedling nurseries and transplanting seedlings. The staggered harvests of amaranth are underway, and their sale also contributes to increasing the savings of the SHGs. These 389 households comprise 3,146 project beneficiaries who are registered in the project database. FSC-R1 activity 3: Prevention of malnutrition: awareness and promotion of good nutritional practices in Health center, Communities and school With the support of 10 community relays, the project continues to organize awareness-raising activities on infant and young child feeding during CPS, CPN and CPON activities at the Nemba health centre and in the community in order to combat malnutrition. During this quarter, the project raised awareness among 219 pregnant and breastfeeding women and 3 child carers. The sub-themes developed were the window of opportunity, optimal nutrition, 4-star nutrition, essential good practices in nutrition, complementary feeding, etc. The community relays also organized 107 home visits with awareness-raising messages. At the time of reporting, the project had already reached 5,891 beneficiaries, including 3,629 in Fizi and 2,262 in Masisi. During this quarter, the project continued to support the agricultural activities of beneficiary households and school gardens, including awareness-raising on sustainable soil management. Community agronomists continued to monitor community and individual fields as well as schools. A total of 2,079 beneficiaries from 300 households supported by the project were involved in the activities organized during this quarter in Masisi (new beneficieires). Similarly, 2,943 beneficiaries from Fizi, grouped into 20 SHGs, received agricultural inputs to continue boosting their agricultural activities in the area. Each SHG received 2 kg of amaranth seeds to sow in their community fields. 358 households each received an additional 50 g of amaranth and each SHG received 1 kg. These beneficiaries also receive local technical support from agronomist monitors. The project also continues to support the AVET activities of young people involved in agriculture in the Fizi area. At the period of reporting, the project had already reached 7,970 beneficiaries, including 3,629 in Fizi and 4,341 in Masisi. The increase in the number of beneficiaries in Masisi is linked to the expansion of the project into new areas and the targeting of 2,079 new beneficiaries during this quarter. During this quarter, the project continued to work with the same 7,970 beneficiaries reached in the third quarter, namely 3,629 in the Fizi health zone and 4,341 beneficiaries in the Masisi health zone. Subtheme Localization Please mention here the total number of national and local actors that received capacity-building support during implementation. In case more than one individual from a national/local actor participate in the capacity building activities, please only count this actor once. Note that this does not include local offices of INGOs. Local partners ( HCC and ADED) and local actors of HaC consisting of health zone employees, the agricultural inspectorate, the division of primary and technical education, the division of social affairs, the division of gender, family and children, local leaders will receive different capacity building support. (20 government workers, 180 members of water committees, 100 children members of school hygiene committees, child protection committees and 20 local leaders ). A total of 28 people were trained on the roles and management of water points. Each committee is made up of 7 people and a village chief. In this training, 2 health committee presidents and 2 full-time nurses were involved Subtheme Food security and livelihoods Basic food needs: assistance, combined with beneficiaries' own resources, to international standard of 2100 kcal per person per day for the period of the action. Any kind of transfer modality (in-kind, voucher, cash) and combination thereof with objective to ensure immediate access to the necessary food commodities. This may include supplementary foods provided alongside general distribution for vulnerable people. Masisi and Fizi Health Zone: 2024-2025 FSC-R2 activity 1: Assessment of CBA infrastructure/feasibility in the region and rapid market assessment FSC-R2 activity 2: Identification and registration of 1000 household according to vulnerability criteria via the 121 platform FSC-R2 activity 3: Purchase and distribution of 1000 basic phones and SIM cards to household FSC-R2 activity 4: Registration of 1000 households to the mobile money service FSC-R2 activity 5: Training of 1000 households on the CVA program and the operation of the mobile money service FSC-R2 activity 6: Provide three round of cash for food to 1000 households (each round 50$) (Year 1: 700HH, Year 2: 300HH) FSC-R2 activity 7: Post distribution Monitoring During this quarter, the project distributed cash to 371 households in the Fizi area, representing 3,062 beneficiaries (912 girls under the age of 18, 993 boys under the age of 18, 594 women over the age of 18 and 563 men over the age of 18) and 380 households in the Nyiragongo area as part of the modified crisis, representing 2,301 beneficiaries (689 girls under the age of 18, 752 boys under the age of 18, 469 women over the age of 18 and 391 men over the age of 18). Households in Fizi each received USD 50 as the second of three rounds planned by the project, while households in Fizi each received USD 75. In total, the project distributed cash to 5,363 beneficiaries (1,601 girls under the age of 18, 1,745 boys under the age of 18, 1,054 women over the age of 18 and 954 men over the age of 18). All these households also received phones and SIM cards, which facilitated the cash transfer. The distribution of cash to households in Masisi is planned for the next quarter. During this quarter, Help a Child distributed the cash to 371 households in Fizi (out of the 400 planned) comprising 2997 individuals (904 girls under 18, 967 boys under 18, 547 women over 18, 559 men over 18, 7 girls under 18 living with disabilities, 10 boys under 18 living with disabilities, 18 women over 18 living with disabilities and 19 men over 18 living with disabilities. Each of these households received a first instalment of 50 USD in their Vodacom M-Pesa account, through the 121 platform. 29 households did not receive the first instalment, and the second instalment is scheduled for distribution in Q2, including the 29 households that did not receive the first instalment due to problems with their Vodacom SIM cards. During this quarter, the project continued to work with the beneficiaries reached during the previous quarter, namely: 5,354 beneficiaries (1,601 girls under 18, 1,745 boys under 18, 1,054 women over 18 and 954 men over 18) and 2,262 new beneficiaries in Masisi who received cash in October (308 girls under 18 and 12 girls under 18 living with a disability, 366 boys under 18 and 5 boys under 18 living with a disability, 746 women over 18 and 30 women over 18 living with a disability, 755 men over 18 and 30 men over 18 living with a disability). At the period of reporting, the project had already reached a total of 7,625 beneficiaries (3,062 in Fizi, 2,301 in Nyiragongo and 2,262 in Masisi). During this quarter, awareness campaigns on birth registration organised in the health areas of Nemba and Sebele reached 1,078 beneficiaries (83 girls under the age of 18, 10 boys under the age of 18, 914 women over the age of 18, and 71 men over the age of 18), including 626 beneficiaries in Sebele and 452 in Nemba. This figure includes the 547 beneficiaries reached during the first half of the year. At the period of reporting, the project had already reached a total of 7,068 beneficiaries under this indicator. Awareness campaigns on birth registration continue in the health areas of Nemba and Sebele. During this quarter, a total of 691 people were reached in Sebele and Nemba (including 588 women, 18 men, 76 girls and 9 boys), broken down into 217 women, 49 girls and 2 men in Sebele, and 371 women, 16 men and 27 girls in Nemba. At the period of reporting, the project had already reached a total of 7,068 beneficiaries under this indicator. Operations staff will focus more on financial management, administration, logistics and procurement. Also once a year learning workshops will be organized to review and redesign activties if needed The crisis modifier has been reactivated to respond to the influx of needs of project beneficiaries who had moved from Goma to Nyiragongo, Masisi and Rutshuru following the dismantling of IDP camps in and around Goma by the M23 when they took control of the city of Goma and Masisi territory. As part of this 3-month crisis modification, Help a Child and its partner ADED are successively implementing activities in the Multipurpose Cash and WASH sectors. Following the training followed in AVET and based on the very volatile security situation in the Masisi territory, we conducted some technical analyzes and direct consultation with the technical advisor of Help a Child for youth and work strategies. The analysis demonstrated that it would be difficult to implement these youth resilience activities at the moment. Thus, we opted for the strategy of relocating part of these activities from the Masisi health zone to the Fizi health zone via local partners HCC and ADED. In addition, this strategy will allow, on the one hand, to be realistic in the target because the approach also requires a good level of support on the ground, which takes time; and on the other hand, allow local partners to implement the acquired knowledge of the training followe Nothing to report NL-KVK-41022454-CDRC003-3 Stichting Red een Kind Since January 20, 2025, security in eastern DRC has deteriorated due to M23 offensives in North and South Kivu, displacing over 400,000 people, 41% of whom are school-aged children.The fall of Goma on January 27th exacerbated the crisis, leaving displaced populations in extreme vulnerability sheltering in host families, abandoned buildings, churches, and schools with minimal access to water, food, healthcare, and shelter. Women and children face heightened risks of GBV, exploitation, and community violence, while humanitarian infrastructure is overwhelmed. Help a Child (HaC) will target Goma and Nyiragongo health zones in North Kivu to address urgent humanitarian needs. A February 1, 2025, assessment revealed a near-total collapse of basic services. Electricity and potable water are cut off, forcing reliance on untreated water and increasing cholera risks. Morgues are full, bodies remain on street, and looted humanitarian warehouses hinder emergency efforts. While 60% of displaced people are returning, many remain stranded due to insecurity, financial constraints, and lack of shelter. Priority needs that Help a Child and local partners ADED and Collectif Alpha Juvi (CAU) will address persist in food security, requiring access to food through MPC to prevent malnutrition. WASH, preventing diseases and improving sanitation with emergency latrines, hygiene awareness campaigns, and WASH/Hygiene kits including menstrual health items. Besides that, Protection concerns are critical, particularly for women and children vulnerable to gender-based violence (GBV) and human trafficking, necessitating safe spaces, psychological support, and family reunification mechanisms for unaccompanied children. Beyond immediate relief, we aim to enhance crisis response capacities and implement return and reintegration strategies, ensuring safe, dignified returns and restoring essential services in host communities. ADED has constructed 50 emergency latrines & shower doors for 50 households, and accessible for people with disabilities. We conducted a headcount of household members utilizing the latrines to establish the precise number of beneficiaries Subtheme Multi-purpose Cash Unconditional transfers are provided to beneficiaries without the recipient having to do anything in return in order to receive the assistance . Unrestricted transfers can be used entirely as the recipient chooses i.e. there are no restrictions on how the transfer is spent (Definitions of CALP). It can be delivered e.g. through local banks, shops, traders, local money transfer companies, remittance companies and post offices in either one or several instalments. Please include in the activity description the amount and frequency of cash transfers (i.e. how often). In case beneficiaries receive multiple transfers (e.g. each month), please only count the beneficiaries once. 3 times cash distributions of 70$ per distribution per household for 670 households (5360) people) using the 121 mobile cash platform, including market assessment and post distribution monitoring. Based on the return movement of displaced people and rapid needs assessments conducted in the Nyiragongo health zone, during the launch of the ACM lead project by Tearfund, it was recommended that Help a Child and its partners ADED and CAU relocate project activities to the Kingarame and Kasizi health areas in the Nyiragongo health zone to avoid duplication of activities during project implementation.Initially, 1,808 households were selected and identified in the Kingarame and Kasizi health areas in the Nyiragongo health zone based on vulnerability criteria proposed by the program's technical team. The targeting criteria were then discussed with a representative sample of community leaders (village chiefs, other community leaders, and members of both sexes) during a community meeting organized in Kingarame and Kasizi health areas Nyiragongo health zone to validate the targeting criteria. Once the Vodacom numbers are assigned, 1808 beneficiaries were identified, these numbers were inserted into the 121 platform. After testing the functionality of these numbers via a 121 message, the technical team finally paid $75 per household to 1808 , or 1,0862 people. Subtheme WASH Hygiene promotion/awareness raising activities. Could be training, campaigns, radio broadcasts, flyers etc. Excludes the distribution of Hygiene/dignity kits that should be covered under WHS-I1. Hygiene awareness for 30.000 people including:training of community relays to mobilize and sensitize communities on good hygiene practices, establishing hygiene committees to promote community involvement in improving sanitary conditions and preventing water-related diseases, supporting the operation of the health zone central office for better coordination of WASH interventions and training women and girls of childbearing age in menstrual cycle management. ADED has conducted the Hygiene awareness for WASH good practices promotion and has reached 14.103 people ADED has trained 60 community relays to mobilize and sensitize communities on good hygiene practices, ADED has established and trained 6 hygiene committees (3 at Kingarame & 3 at Kasizi) to promote community involvement in improving sanitary conditions and preventing water-related diseases. ADED has supported the operation of the health zone central office for better coordination of WASH interventions ADED has trained 600 women and girls of childbearing age in menstrual cycle management. Creation of Mobile Child Friendly Spaces (CFS) for psychosocial and educational support of 10.800 children. The partner CAU established fixed and mobile child-friendly spaces in public schools in the Kasizi and Kingarame health areas of the Nyiragongo health zone. These spaces provided opportunities for awareness-raising sessions on children's rights and responsibilities, personal hygiene, the importance of education and environmental protection, child protection, family unity, and separation prevention. Other activities were organized within the community by two local protection committees (Kasizi and Kingarame), each composed of 10 members, at two sessions per week. Subtheme Protection 1) Care arrangements can include temporary care arrangements in host family or other care arrangements.2) Appropriate means (but is not limited to) that care arrangements must take into account different needs of children based on their age, sex and diversity. Morevoer they must as much as possible be defined taking into account the specificities of the context and - when appropriate - be built on existing coping mechanisms. 3) Appropriate is related to the physical safety and psychosocial well-being of the child. Specific attention should be put on non-discrimination of fostered children compared to biological ones. 4) Unaccompanied and/or separated children must be disaggegated by age and sex. This indicator excludes the reunification of other family members (above 18) which is to be reported under PRO-R14 Identification, case management and reunification of 60 Separated and Unaccompanied Children (IDTR) including temporary foster care. As part of the Indification Tracing and Réunification process, 10 community members, including 4 Parasocial Workers, 2 Community Relays, 2 Community PrAs part of the Identification, Documentation, Tracing, and Reunification process, 10 community members, including 4 community parasocial workers , 2 members of the Community Protection Network, and 2 community leaders, were trained on the Identification, Documentation, Tracing, and Reunification (IDTR) process for separated children (ES) and unaccompanied children (ENA). The Collectif Alpha Ujuvi supported 10 foster families (FAT) and 60 separated and unaccompanied children (37 unaccompanied children, 13 separated children, and 10 child heads of household) who had specific needs. These children benefited from socio-economic and educational reintegration, including 60 children (F39 and G21) who received 60 clothing kit, 40 received a school reintegration kit, and 20 child parents received a socio-economic reintegration kit Protection Networks, and 2 Community Leaders, were trained on the Identification, Documentation, Tracing, and Reunification (IDTR) process for separated children (ES) and unaccompanied children (ENA). Furthermore, structured feedback mechanisms, including complaint and response systems will help to ensure that affected people can continuously provide input and raise concerns. A mid-term learning workshop will be conducted after three months to assess progress and adjust activities as needed, ensuring that the program remains responsive to the evolving needs of the communities. At the start of the project, we will organise community consultations with 150 community representatives in each health area to select the feedback mechanisms that will be put in place, i.e. 450 community members for the 3 health areas. During the implementation of the project, we will organise community accountability meetings with these community representatives, during which we will provide responses to their complaints and feedback. In addition, we will set up a local accountability committee (AAP committee) in each health area. Each committee will have 9 members, including 1 girl child, 1 boy child, 4 women and 3 men. As part of this emergency declaration, a joint InterAgency Flash Appeal developed by UNHCR in coordination with the Government and Partners was released, with eight partners appealing for a combined US$33.2 million to respond to 90,000 new arrivals over a four-month period. UNHCR has launched a four-month emergency appeal for the period December 2025 through March 2026. Under this appeal UNHCR is requesting US$47.2 million to support the immediate and ongoing response to IDPs in eastern DRC, reinforce preparedness, and sustain life-saving protection and assistance for refugees and returnees arriving in Burundi as well the broader region. Priority sectors: WASH, Shelter/NFIs, Protection and multipurpose cash assistance (MPC) as a modality to meet the most urgent needs of displaced families. Beneficiaries will be targeted with a combination of activities from different sectors. The topics discussed during the dialogue are gender equity, gender roles, power dynamics in the community on it influence food security and livelihood. ACROSS established 22 project management and accountability committees comprising 8 females and 14 males, including 1 female and 3 males with disabilities. These members play a critical role in designing the project, as well as in its implementation and monitoring. A total of 242 complaints/feedback were received, of which 106 were from men,54 from women,26 from girls,30 from boys,10 from men with disability,8 from women with disability,6 boys with disability and 2 girls with disability.200 individuals participated in various activities conducted in Pibor in Q1 consisting of 98 females,55 males,22 girls,20 boys,1 female with disability,2 males with disability and 2 girls with disability. March 25th, 2024, the SSJR 2024-2026 project was initiated and launched in Pibor by the four SSJR partners for Pibor, namely ACROSS, ZOA Dorcas, CARE, and Help a Child. The launch was attended by government ministry officials and members of the community. A total of 56 individuals, comprising 16 females and 40 males, participated in the event. School debate were conducted in Pibor Administrative area many people attended an a reached with key messages on GBV and child marriage were about 140 girls and 170 boys, parents male 28 and female 32, persons with disability were about 12 female 3 and male 9. What’s Up Children– PSS 30 children 12 boys and 18 girls in Tonj North and 16 boys and 16 girls in Pibor. Any kind of transfer modality (in-kind, voucher, cash) and combination thereof with objective to ensure immediate access to the necessary food commodities. This may include supplementary foods provided alongside general distribution for vulnerable people. Component 1 - IPC5 Famine Response (crisis modifier): food aid to 6,000 new people (600 HH). March 25th, 2024, the SSJR 2024-2026 project was initiated and launched in Pibor by the four SSJR partners for Pibor, namely ACROSS, ZOA Dorcas, CARE, and Help a Child. The launch was attended by government ministry officials and members of the community. A total of 56 individuals, comprising 16 females and 40 males, participated in the event. - The identification and registration of beneficiaries for seeds and tools support took place in Akilo and Verteth. In Akilo, 200 households were registered for this support, with a breakdown of 392 girls, 375 boys, 224 females, and 200 males. The beneficiaries were mobilized for registration on April 19th, 2024, and the actual registration occurred on April 23rd, 2024. Similarly, in Verteth, 200 households were registered, consisting of 402 girls, 365 boys, 239 females, and 185 males. The beneficiaries in Verteth were mobilized and registered on April 20th and April 22nd, 2024, respectively. - A women vegetable production group in Pibor achieved a successful yield and harvest of various stable vegetables between February and March. The group, which is comprised of 30 female members, cultivated cowpea leaf, Amaranthus, Jew mellow, and okra. Their efforts resulted in a bountiful harvest of these vegetables, contributing to the overall agricultural productivity in Pibor. Out of the 11,280 individuals reached, 2,267 were females and 2,204 were males individuals under 18 years, while 4,092 were females and 2,352 were males individuals above 18 years, 10 were females and 17 were males under 18 years with disabilities, and 208 were females and 130 were males above 18 years with disabilities. ACROSS trained 30 women in food value addition, with a focus on improving their skills in fish handling, processing, and preservation, as well as honey processing, packaging, and managing waste. A total of 3,600 individuals benefited from the distribution of seeds and tools, including 61 females and 20 males under 18 years, 2,311 females and 895 males above 18 years. 2 females and 5 males were under 18 years with disabilities, and 186 females and 120 males were above 18 years with disabilities. Subtheme Food security and livelihoods Resources that enable people to protect and rebuild their livelihood assets include seeds, livestock, tools, business grant, etc. Any kind of transfer modality (in-kind, voucher, restricted cash) and combination thereof to support, protect and enable the restoration/protection/access of/to livelihood assets. • Distribution of agricultural inputs, including seeds and tools to support vulnerable households and ensure sustained agricultural productivity. • Training of farmers covering a range of agricultural practices, from crop and vegetable production to fish preservation and beekeeping initiatives, ensuring farmers are equipped with the latest knowledge and skills. • Support to vulnerable groups, providing business training, coaching, and financial assistance. • Formation of groups and committees, from farmer field schools to marketing and natural resources committees. • Environmental and hazard-related activities like the establishment of demo-farms, support for storage facilities, and engagement in hazard risk analysis. • Activities such as cooking demonstrations and bi-annual post-harvest assessments. Out of the 11,280 individuals reached, 2,267 were females and 2,204 were males individuals under 18 years, while 4,092 were females and 2,352 were males individuals above 18 years, 10 were females and 17 were males under 18 years with disabilities, and 208 were females and 130 were males above 18 years with disabilities. ACROSS trained 30 women in food value addition, with a focus on improving their skills in fish handling, processing, and preservation, as well as honey processing, packaging, and managing waste. A total of 3,600 individuals benefited from the distribution of seeds and tools, including 61 females and 20 males under 18 years, 2,311 females and 895 males above 18 years. 2 females and 5 males were under 18 years with disabilities, and 186 females and 120 males were above 18 years with disabilities. Furthermore, 7,200 individuals benefited from seed distribution under the crisis modifier in Rukon Juba County, with 2,150 females and 2,134 males individuals under 18 years, and 1,739 females and 1,156 males above 18 years. 4 females and 5 males were under 18 years with disabilities, and 5 females and 7 males were above 18 years with disabilities. Additionally, 300 individuals received fishing equipment, including 2 females and 9 males under 18 years, 21 females and 265 males above 18 years, with 3 males above 18 years having disabilities. Lastly, 150 school children participated in agricultural training, with 54 females and 41 males under 18 years, and 3 females and 36 males above 18 years. Among the participants, 4 females and 7 males were under 18 years with disabilities, and 5 females were above 18 years with disabilities. Subtheme Food security and livelihoods Basic food needs: assistance, combined with beneficiaries' own resources, to international standard of 2100 kcal per person per day for the period of the action. Any kind of transfer modality (in-kind, voucher, cash) and combination thereof with objective to ensure immediate access to the necessary food commodities. Conditional cash transfers can include cash for work, cash for assets and cash for training. The 37,24% is calculated: the Total local implementing partner costs (so incl ICR) divided by the Grand Total of all the Member projects (so not the total lead budget). That is 7.832.761 EUR* / 21.032.291 EUR **. The campaigns aim to reach influential leaders, policy enforcers, and policymakers, with specific strategies for each campaign approach. One School debate was conducted in Pibor where approximately 282 individuals were reached with key messages on GBV and child marriage, including 410 children(140 girls and 170 boys), 60 parents(male 28 and female 32) and 12 persons with disabilities(female 3 and male 9). Two child protection campaigns were conducted in Pibor and Tonj North to reduce issues of early child marriage through boruboru. Three protection campaigns were conducted in Pibor Akile, Verteth and Gumuruk payams during FSL distribution by ACROSS. Out of these, 96(54M,42F) were PTA members,30(16M,14F) were CPC members,14(8M,6F) were members of centre management committee, 127(33M,94F) were members of Whats up people,60 were members of Whats up ladies,8 (7M,1F) were staff from MGCSW,158(43M,115F) were parents who attended BoB sessions,4(2M,2F) were counsellors and 11(3M,8M) Parents participated in FGDs. Subtheme Education in emergencies INEE Standards include: 1) accessibility, regardless of physical ability; 2) Clear marking of the learning environment; 3) appropriate physical structure and space; 4) space promoting learner-centred approaches, 5) community engagement, 6) basic health and hygiene, 7) adequate sanitation, 8) availability of sufficient, safe drinking water. • Procurement and installation of ECD playing equipment, focusing on providing resources for interactive learning. • Repair and maintenance of playing equipment. • Establishing/rehabilitating ECD temporary learning spaces (TLS). • Establishing/rehabilitate climate-resilient semi-permanent learning spaces (TLS) in schools. • Providing school inclusive learning desks • Constructing gender-segregated & inclusive climate-resilient WASH Facilities: • Erecting protective fences around schools and TLS • Establishing rainwater harvesting systems and water points in schools • Rehabilitation and support of primary schools In Tonj North 2 TLS were constructed in 2 primary schools selected in Tonj North, but no TLS construted in Pibor . In 2024, 28496 individuals were reached with 7 campaigns including 21007(8009 male, 7550 female,2566 boys,2818 girls and 64 PwDs) reached through Child rights campaigns,2523(462 male,1083 female,526 boys,441 girls and 11 PwDs) reached through Zoom out campaigns, 667(252 male,400 female and 15 PwDs) reached through CP activities during FSL and 4299(43 male,19 female,1996 boys and 2241 girls) reached through Debate. Subtheme Protection 1) Care arrangements can include temporary care arrangements in host family or other care arrangements.2) Appropriate means (but is not limited to) that care arrangements must take into account different needs of children based on their age, sex and diversity. Morevoer they must as much as possible be defined taking into account the specificities of the context and - when appropriate - be built on existing coping mechanisms. 3) Appropriate is related to the physical safety and psychosocial well-being of the child. Specific attention should be put on non-discrimination of fostered children compared to biological ones. 4) Unaccompanied and/or separated children must be disaggegated by age and sex. Establishing peace committees and peace clubs for reconciliation and peace-building activities. • Recruiting, training, and engaging GBV case workers to provide case management services and legal counseling to GBV survivors. • Establishing a women and girls friendly center and distributing psychosocial support materials. • Pibor: Organizing child rights protection campaigns to reduce issues of early child marriage in Pibor through Borubour number of children and adult estimated were 6894, Men were 570, women were 145, boys were 2948 and Girls 3200, women with disability 0, men with disability 12, boys with disability 10 and girls with disability 9. Tonj North: Organizing child rights protection campaigns in Tonj North through Borubour number of children and adult estimated were 18092, Men were 1266, women were 1318, Boys 7480 and Girls 7964, 20 girls with disability, 18 boys with disability, 14 men with disability and 12 women with disability. Subtheme WASH Regular and timely access: 250g soap/p/m for personal hygiene, 200g soap/p/month for laundry. This includes hygiene/dignity kits. Access may be in-kind or though voucher distribution or through unconditional restricted cash to enable beneficiary to buy soap (without compromising access to other basic needs). Distributed items need to be culturally acceptable i.e. take into account local practice and expectations. 400 Households in Pibor receive hygiene kits Subtheme WASH Access: ratio (user/facility) of 1 toilet for a max. 20 people, distance < 50 metres from dwellings, or as locally agreed. Clean: regular cleaning and maintenance for public facilities. Dignified: Use of toilets respect cultural preference and is arranged by household(s) and/or segregated by sex. Unsafe facilities: unstable (unlined) pits with risk of collapse, pits accessible to vectors, pits contaminating water tables and poorly sited facilities which expose women and girls to attacks, especially at night. Functional facility: fully constructed, in working order and properly maintained, of a type and in a location acceptable to intended users, with hand washing facilities and anal cleansing material. • Rehabilitation of institutional latrines. • Training of PHAST promoters and frontline staff. • Support the Participatory Hygiene and Sanitation. Transformation (PHAST) promoters who are engaged in promoting hygiene and sanitation practices. • Community sensitization and mobilization. • Conduct domestic waste management campaigns, addressing the proper handling and disposal of waste within communities. • Training of masons and carpenters on latrine construction/rehabilitation, using locally available materials • Production of IEC materials, kits, and visibility products Subtheme Nutrition Community screening / MUAC. In case of multiple screenings during the timeframe of the project, the PLWs and children should only be counted once. Provide community outreach and mobilization (early screening, referral, preventive services, including improved Maternal Infant and Young Child Nutrition Practices - MIYCN practices) to resident, returnee, and IDP children under five, as well as pregnant and lactating women. Please note that response to GBV is captured under PRO-I12. • Strengthen, equip and refurbish women and girls friendly spaces (WGFS) for psychosocial support and resilience programs, with a focus on building community protection networks to mitigate gender-based violence (GBV) risks, providing access to quality specialized GBV services. • Support GBV mitigation and response activities, encompassing psychosocial support, counseling, information sharing, awareness raising, and skills-building. • Providing MHPSS Level 3 and 4 services to identified adolescents and their families, including community outreach on available services, distribution of MHM kits to vulnerable girls and matrons in schools, and training on the production of locally made reusable sanitary pads for the most vulnerable girls and matrons. The primary focus is on re-establishing and maintaining contacts and reunification with family members, which includes the registration of missing persons and ongoing communication with their families. The goal is to support the family tracing and reunification process for unaccompanied and separated children. Subtheme WASH Access: maximum distance to water point 500m, queuing time less than 15min, filling time maximum 3 min/20 litres or as locally agreed. Water access should be during the whole period of implementation unless action is mostly related to building the local water delivery capacity. Sufficient: covering basic needs, i.e. 7.5-15 l/p/d or as locally agreed. Safe: 1) low risk of faecal contamination, 2) No faecal coliforms detectable in any 100-ml sample, 3) For piped water supplies, or for all water supplies at times of risk or presence of diarrhoea epidemic, water is treated with a disinfectant to achieve free chlorine residual at the tap of 0.5 mg per litre and turbidity is below 5 NTU, 4) If for a short period, water which is contaminated chemically or radiologically is used, no (significant probability of) negative health effect is (likely to be) detected. Subtheme Protection 1) This indicator covers response to survivors of GBV (women, men, girls and boys).2) Appropriate response is defined as the provision of comprehensive services/assistance based on a thorough analysis of the risks faced by persons. The protection outcome of the response must be well defined. Subtheme Multi-purpose Cash Unconditional transfers are provided to beneficiaries without the recipient having to do anything in return in order to receive the assistance . Unrestricted transfers can be used entirely as the recipient chooses i.e. there are no restrictions on how the transfer is spent (Definitions of CALP). It can be delivered e.g. through local banks, shops, traders, local money transfer companies, remittance companies and post offices in either one or several instalments. Please include in the activity description the amount and frequency of cash transfers (i.e. how often). In case beneficiaries receive multiple transfers (e.g. each month), please only count the beneficiaries once. • Provision of multi-purpose cash to the most vulnerable households unable to work to meet basic household requirements. Participation does not take place when activities are carried out in exchange for a salary of 'in-kind' payment unless the affected people are involved in decision-making processes and have an impact on decisions that affect them. Please describe in the actvity description what this involvement specifically entails and in which phase of the program conflict-affected people are involved. Medair and local partners will be keen to engage target communities including beneficiaries, community leaders, women leaders village committees and local authorities from Karan and Daynile to be involved at the initial needs assessment. Quarterly meetings will be held during implementation to monitor, discuss progress, possible adjustment based on identified gaps through dialogue with the affected communities. The project will target a total of 606 people to participate each year. During the reporting period, 554 crisis-affected people were involved in the design, implementation, monitoring and/or evaluation of the programme, against a target of 606, equivalent to a 91.4% achievement. During community engagement meetings, participants provided feedback to the project team and acknowledged the progress and performance of the agreed project scope. In 2025, a total of 374 participants including 119 males, 255 females, and 12 persons with disabilities (PWDs) took part in community engagement and consultation meetings facilitated by the project staff. This represents 61.7% of the annual target of 606 participants, showing significant progress compared to the previous report, where achievement stood at only 18.5%. The project acknowledges this improvement as a strong indication of strengthened integrated coordination across all sectors and enhanced stakeholder involvement at every stage, both within Medair and among partners. Continued efforts and upcoming implementation activities are expected to further accelerate progress to ensure the annual target is fully achieved. Subtheme Health Skilled health personnel: doctors, nurses or midwives trained in providing life-saving obstetric care, including in giving the necessary supervision, care and advice to women during pregnancy, childbirth and the postpartum period, and trained to conduct deliveries on their own, and to care for new-borns. In case this concerns trained Traditional Birth Attendants, please state this explicitly in your activity description. Support 24/7 maternity services (skilled, antenatal clinics, postnatal visits, vaccination for pregnant women as well as newborn care) at 2 health facilities in Karan District. During the 3-year implementation period, it is expected that 13,925 pregnant women will be assisted in delivering safely by trained health professionals. The project anticipates implementing this activity continually throughout the three years in the same HFs. Refresh training for 12 Midwives on BeMONc and Essential Newborn care by Medair every year during the 3-year project period. In the first year of the project, 4,244 successful deliveries were recorded, including 50 girls under the age of 18. These deliveries occurred at the two supported health facilities, Jamhuriya and Negeyle, in Karan district, and were attended by skilled health professionals. This achievement represents 30.5% of the project's overall target of supporting 13,925 live deliveries at the two health facilities over three years, with a first-year performance reaching 91.4% of the annual target. In 2025, the project has supported a total of 3,806 safe deliveries at Jamhuriya and Negeyle health facilities in Karan district. This includes 54 deliveries by girls under 18 and 9 by female adults with disabilities. All deliveries were attended by skilled health personal. Year 2 achievement shows a 10% decrease compared to Year 1 (4,244 deliveries). The reduction is mainly due to the scale-down of WFP-supported preventive nutrition activities that attracted beneficiaries from outside the catchment area. It is also linked to the temporary closure of one health facility for three weeks because of relocation. Subtheme Health Number of curative consultations provided either through existing facilities which received substantial support through the project or through parallel, self-standing emergency facilities. Support for free outpatient curative consultations at two supported HFs in Karan. To ensure the quality of the offered PHC services, these HFs will be supported with the procurement and supply of essential medicines and other medical equipment, staff incentives, capacity building in clinical skills for health staff, technical support through routine supervision by Medair Health supervisors, and joint supportive supervision with the Ministry of Health. The project conducted 51,706 primary health consultations at the 2 supported health facilities (Jamhuriya and Negeyle) in Karan district during the period under review, which is equivalent to 70.8% of the project's annual target of 73,007 consultations. The performance was impacted by the temporary closure of Jamhuriya health facility and suspension of services. In 2025, the project continued delivering outpatient curative consultations at the 2 supported health facilities in Karan District, providing a total of 50,850 consultations. Subtheme Localization This concerns capacity building such as training and on the job support to national and local actors. This is captured under "Capacity building local partners" in the DRA budget. Please divide Capacity building local partner costs by the Grand Total Budget (incl. ICR) to calculate this percentage. Note that this does not include local offices of INGOs. Implementing partners will utilize 5% of their direct costs for capacity strengthing on priority areas they identified (systems development) during the year 3 project period. ZAMZAM and DAWA want to acquire ERP systems for finance, procurement, human resources, payroll, and inventory management. The ERP system will handle transaction cycles from requisition to voucher payment. Consultants will oversee system installation and train key staff over the first two years. DAWA will create a website and focus on communications in year 2, while in year 3 they will work on performance management policies. Zamzam will update policies and strengthen their organization's capacity through ethics-related trainings in years 2 and 3, with donor co-funding. SAACID will utilize its 5% to establish a hotline to receive complaints, acquire HR and procurement software and hire a consultant to carry out an organisational capacity assessment. Medair will also support local partners with coaching on the job and internal trainings where needed and Help a Child will support with capacity building and coaching on Protection. At least 5% of the total direct costs will be spend on capacity building. This also includes capacity building of local staff. Medair and its 3 local implementing partners (DAWA, SAACID, and ZAMZAM) attended an in-person workshop on Capacity Strengthening convened by the SOMJR Coordinator and attended by other SOMJR partners during Y1 of the project. During the meeting, all partners presented their respective priority capacity assessment gaps. All 3 local implementing partners identified gaps in their Human Resources, Procurement, and Finance systems, and proposed the purchase of an Enterprise Resource Planning (ERP) software as a solution. Following the meeting, both Zamzam and DAWA received their capacity-strengthening budgets after submitting detailed plans outlining how the funds would be utilized for ERP software installation. Due to their smaller entitlement, SAACID will utilize Y1 and Y2 budget allocations combined to implement some of the identified ERP systems gaps. Budget for SAACID will be disbursed in Q2 in Y2. They have also asked whether there is any possibility of additional funding support (top-up) from Medair. Medair and its 3 local implementing partners (DAWA, SAACID, and ZAMZAM) attended an in-person workshop on Capacity Strengthening convened by the SOMJR Coordinator and attended by other SOMJR partners during Y1 of the project. DAWA implemented HR and Procurement Modules of its ERP system in Year 2 (following Finance and Project Management Modules implemented in Year 1), including configuration, data migration, reporting and dashboard development, and staff training. SAACID successfully installed and configured its ERP project management system, integrated it with the finance module, and launched operations. Zamzam utilized its allocation for system configuration and data migration. Subtheme Nutrition Total number of cases admitted in nutrition program during the timeframe of the program. Children which are admitted to MAM treatment after SAM treatment should be counted only once. Procurement and supply of nutrition supplies for the treatment (rehabilitation) of 24,303 malnourished children (6628 SAM and 17675 MAM). The nutrition supplies will cover all 12 health facilities and 22 outreach locations supported by Medair in Banadir, Hirshabele, Southwest, and Lower Juba regions. Calculation of this indicator was revised to include only nutrition admissions in health facilities which received DRA-funded supplies in 2024 and to remove admissions supported by WFP supplies. In 2025, the project continued providing lifesaving treatment for acute malnutrition at supported health facilities, admitting 9,541 children under five (4,113 boys and 5,326 girls), including 102 children with disabilities Subtheme Nutrition Nutrition programme at health facility level: presence of trained personnel, adequate equipment, adequate supplies and management of cases. Focus is on the functionality of the nutrition programme independently of the “use of the service” by the target community. Support the implementation of the CMAM programme (community mobilisation, OTP, and TSFP) in 3 static health facilities in Banadir (Caawiye, Neygele, and Jamhuria) and, as well as the procurement of nutrition supplies buffer stock in an additional 9 static health facilities (Dameerale, Galoley, Bulo Karamo, Shagallaha, Abdalla Birole, Burgabo, Ras Kamboni, Kudhaa, and Gobweiyne) and 22 outreach locations (11 in Balcad, 2 in the Afgoye corridor, 9 in Cadale). 10 healthcare staff will be trained in the prevention and management of acute malnutrition (CMAM). Referral of 200 SAM cases with medical complication to stabilization centre for inpatient care. The project provided nutrition services at 33 sites during Q3 and Q4. These included: • 3 static health facilities in Banadir (Caawiye, Neygele, and Jamhuriya) • 7 static health facilities (Damerale, Galoley, Bulo Karamo, Shagallaha, Abdalla Birole, Ras Kamboni and Gobweyne) supported with the procurement of nutrition supplies • 21 outreach locations in Cadale and 2 outreach sites in Bardheere. Support for Burgabo and Kudhaa health facilities in Badhaadhe district ended in Q2 when Medair handed over the running of the facilities to the local implementing partner. After this, a new health facility was supported in Baardheere district (Gedo Region) in Jubaland. In 2025, nutrition services were provided at 31 sites: 3 fixed sites in Mogadishu with full CMAM support by REK; 8 additional fixed health facilities supported with nutrition supplies (Bulakaramo HF in Cadale, Gololey and Qalimow HFs in Balcad, Damerale HF in Afgoye, Shaqalaha, Gobweyne, and Abdalla Birole HFs in Kismayo, and Raskamboni HF in Badhaadhe); and 21 outreach sites in Cadale. Support to the 21 outreach sites in Cadale ended in March 2025 due to access challenges, and support to 3 fixed health facilities -Abdalla Birole, Galoley, and Qalimow concluded in August 2025 due to reduced US state/OFA funding support. Subtheme Protection 1) This indicator covers response to children (girls and boys under 18). 2) Appropriate response is defined as the provision of comprehensive services/assistance based on a thorough analysis of the risks faced by persons. If relevant, please specify in the activity description your target population, for example child soldiers. Implementing and continued strengthening the existing child friendly space at Nageyle, Karaan with equipment accessible to all children including those identified with disabilities.The project will target 3,168 children each year A total of 9,504 children will regularly engage in group and free play activities that are accessible to all children over the courses of the 3 years. The CFS will provide children an opportunity to play, grow and develop holistically. It will also provide a context for children to try new social skills and challenging new tasks, and to solve complex problems. It is expected that play will enable children to find their place in their society and culture and provide temporary relief from the hardships they face in day-to-day life. During Year 1, the Child-Friendly Spaces (CFS) activity exceeded its initial target of 3,168, achieving a total of 4,768 participants (2,247 boys and 2,521 girls), including 2 children with disabilities. This represents a performance of 50.5% above the target. However, the project acknowledges the need to improve the CFS data collection process to minimize or eliminate double counting during reporting (i.e. a way to ensure that repeat attendees are not counted every time they attend). During Y2, the child-friendly space (CFS) continued to operate and actively engaged children in free play and recreational activities. The sector worked closely with other health/nutrition service areas, as mothers and caregivers frequently brought their children while seeking health services. Children participated in age‑appropriate play sessions, supported and supervised by trained facilitators. A total of 4,238 children were reached, against a target of 3,168, representing 131% achievement. This figure includes some double counting due to repeated attendance by the same children. Subtheme Health Number of hospitalisations at secondary healthcare facilities which received substantial amount of support through the project. Referrals of severe cases that cannot be treated at supported primary health care facilities and require secondary or tertiary care. These cases will include obstetric emergency cases and SAM cases with medical conditions. The referral support for obstetric emergencies will cover 2 PHC facilities in Karan where 24/7 skilled delivery is supported, while the referral support for SAM cases with medical complications will cover Karan and Daynile. The project anticipates to cumulatively support 670 serious cases throughout the project period. During the reporting period, 163 referrals were made, including 144 for obstetric emergencies and 19 for children under five (CU5), which could not be adequately managed at the supported health facilities. These cases were promptly referred to secondary facilities for higher levels of care, following strict referral protocols. Staff were fully trained to manage cases promptly and minimize referrals. This figure represents 73.1% of the project's Year 1 target of 223 referrals and 24.3% of the three-year target of 670 referrals. This indicator is impacted by underachievement of total consultations. All cases which required referral were appropriately supported. During the reporting period, Medair facilitated 181 emergency referrals to secondary‑level care, including 162 obstetric emergencies and 19 cases for children under five requiring specialized management. Subtheme WASH Average % of the following indicators: - % of population considering that their basic WASH needs are met. - % of population with adequate hygiene practices (according to SPHERE standards on appropriate use and regular maintenance of facilities and on hand washing) Sample size should be based on your specific target group, not on the catchment area. Standard list of questions can be found on indikit.net. Only include questions based on your intervention. For example if you do not construct latrines you cannot ask questions about access to latrines. See activities under results WSH-I1, WSH-I4, and WSH-I6 for detailed descriptions of the activities intended to improve access to safe drinking water and improve hygiene practices.  This will be measured by household survey (PDM) conducted in Daynile after project activities have been completed. The target is based on an assumed average of handwashing practices and access to safe water. Based on the average value between: % of households accessing safe treated water and % of households reporting handwashing with soap for at least 2 critical times (after defecation and one other time) Based on running average values from multiple distribution rounds, in 2025, 90.7% of the target population accessed safe treated water and practiced handwashing with soap at two critical times. This overall performance reflects an average of 96% (CM) and 98.73% (REK) of households accessing safe treated water, alongside 90% (CM) and 77.90% (REK) of households practicing handwashing with soap after defecation and at one additional critical moment. These results demonstrate strong progress toward the expected outcome, with consistently high adoption of essential hygiene behaviors across targeted households under both the Crisis Modifier and regular programming modalities. Subtheme Protection Dimensions of mental health and/or wellbeing include ((as per IASC MHPSS Framework): 1) improved functioning (e.g. the ability to carry out essential activities for daily living); 2) improved subjective wellbeing (e.g. positive emotions, self-concept, life satisfaction, hopefulness); 3)Reduced distress and/or mental, neurological or substance use disorders (or symptoms thereof); 4) Improved ability of people with mental health and psychosocial problems to cope with problems (e.g. through skills in communication, stress management, problem-solving, conflict management or vocational skills); 5) Improved social behaviour (e.g. reduced aggression); 6) Improved social connectedness (e.g. increased quality and number of connections an individual has with other people in their social circles of family, friends and acquintances) To be measured at target group level. % of target persons who report improvement in their mental health and psychosocial well-being as described under PRO-I1 During Year 1 of the What Up Parenting sessions, pre- and post-tests using the WHO-5 Well-Being Index were conducted. However, the project noted data errors and inconsistencies at the Jamhuuriya site, as well as a lower number of participants in Dayniile and Nageyle, which hindered the comparison of pre- and post-test results. Despite these challenges, 68 participants from the What Up Parents Groups at two health facility sites, Caawiye and Negeyle, showed improvements in psychological well-being. Specifically, 69% of participants (47 individuals) demonstrated an improvement. In Jamhuuriya, due to data collection errors and inconsistencies, the data could not be used. However, support will be provided to ensure proper implementation of the tool in the future. In Y2, a total of 80 individuals receiving mental health and psychosocial support (MHPSS) services were assessed using the WHO-5 Well-Being Index at both baseline (1st consultation) and end line (last consultation). The WHO-5 is a standardized tool designed to measure subjective psychological well-being, with scores ranging from 0 to 25. Results indicate that 100% of participants demonstrated improvement, defined as an increase in their WHO-5 score between the two time points. Statistical analysis using a one-sided paired-samples t-test confirmed a significant increase in well-being following the intervention. The overall mean score rose from 7.50 at baseline to 23.30 at follow-up, resulting in a mean difference of 15.80 points. This change was statistically significant (t(85) = 37.11, p < .001), with a very large effect size (Cohen’s d = 4.00), indicating a substantial and clinically meaningful impact of the MHPSS services on participant well-being. All health facilities showed consistent and significant gains. At Jamhuriya, the average score increased from 8.56 to 23.83, with a mean difference of 15.27 points and a corresponding effect size of 6.20. Similarly, participants at Negayle experienced a mean score increase from 6.16 to 22.63, yielding a mean difference of 16.47 points and an effect size of 3.15. In both cases, p-values were less than .001, confirming strong statistical significance. Subtheme Food security and livelihoods Basic food needs: assistance, combined with beneficiaries' own resources, to international standard of 2100 kcal per person per day for the period of the action. Any kind of transfer modality (in-kind, voucher, cash) and combination thereof with objective to ensure immediate access to the necessary food commodities. This may include supplementary foods provided alongside general distribution for vulnerable people. 536 households comprising of 3,674 individuals (1,646 male 2,028 females), including 581 persons with disability received basic food through Crisis Modifier during the project's first year. (DAWA distributed food to 1,897 individuals from 256 HHs in Beledweyne district. Zamzam distributed food to 1,777 individuals from 280 HHs in Garasbaley district in Mogadishu. In 2025, a total of 3,816 individuals from 559 households received food assistance in Garasbaley and Gubadley districts in the Benadir region through the crisis modifier. (2,005 females and 1,811 males) . DAWA assisted 2,224 individuals from 259 households in Gubadley district, including 1,161 females and 1,063 males, while Zamzam supported 1,592 individuals from 300 households in Garasbaley district, comprising 844 females and 748 males. Subtheme Health Number of SGBV victims that receive a comprehensive package of medical and paramedical services within 72 hours of the assault. In case SGBV victims receive medical services after 72 hours of the assualt, please indicate the number of victims supported in the activity description field. Provision of clinical management of rape services, including integragted PSS for SGBV, through the 3 supported HFs in Karan and Deynile ( Nagele HF, Jahmuriya in Karan and Caawiye HF in Deynile) During this reporting year, 7 survivors of SGBV cases, all girls under the age of 18, sought medical care at the Negeyle and Jamhuriya Health Facilities in Karan and Caawiye HF within 72 hours of the assault. 4 survivors in Q4 received clinical management of rape post-exposure treatment, medical care, injury treatment, and psychosocial support, and were referred to a nearby SGBV site for further care. 3 survivors in Q3 received psychosocial suppport and were promptly referred to other facilities for the comprehensive package of medical and paramedical service due to the lack of PEP kits at the facility at that time. The low turnout in the first year is attributed to the community's low uptake of SGBV services and the lack of PEP kits at the supported health facilities during the first two quarters of the year. This equates to 70% of the first-year target and 23% of the overall three-year target of 30 cases. In 2025, 8 survivors of SGBV including two girls under 18 and six women over 18 sought medical care at Negeyle, Jamhuriya (Karan), and Caawiye Health Facilities within 72 hours of the incident. All survivors received clinical management of rape (CMR), including post-exposure treatment, injury management, medical care, and psychosocial support. Note that this does not include local offices of INGOs. Implementing partners run 2 health facilities (Karan, Daynille, including nutrition activities (10 other HF, 22 mobile outreach sites). They engage and pay the staff and utilities bills. In addition, the crisis modifier response activities will be implemented directly by the local partners. They engage in soap distributions during emergencies and water distribution at water kiosks. Budgetary allocation to the partners for running of health facilities, and crisis modifier responses has been fully disbursed, except the capacity strengthening amount allocated to local partner SAACID, for which SAACID was not able to plan for implementation in Y1. instead, they will carry the amount forward to combine with the Y2 allocation and spend both amounts together. Medair's implementing partners (Dawa, and Zamzam) successfully run the respective health facilities: Caawiye health facility in Deynille as well as Jamhuriya and Negeyle health faiclities in Karan districts. The project continued supporting the local implementing partners with timely disbursement of funds to settle staff salaries and utility bills, thus ensuring continuiry of services at the supported facilities. DAWA, Zamzam and SAACID fully utilized their share of crises modifer allocationa and completed their crisis modifier response within the stipulated timelines. Subtheme Localization Please mention here the total number of national and local actors that received capacity-building support during implementation. In case more than one individual from a national/local actor participate in the capacity building activities, please only count this actor once. Note that this does not include local offices of INGOs. Medair Health Facility supervisors based at health facilities will carryout on-the-job traning and coaching for partner health facility staff on daily and on a bi-weekly basis by sector managers. Medair will coodicate with Ministry of health (MoH) and provide other training including IMAM, BeMONc, Rational Use of Drugs, mhGAP-HIG, and waste management etc annually during the 3 year project period. Medair continued to support all three local implementing partners with training and on-the-job mentoring for health facility staff. In case of multiple screenings during the timeframe of the project, the PLWs and children should only be counted once. Screening and referral of children under five (boys and girls) for SAM and MAM management in all 12 HFs sites 114,240 CU5 are expected to screen for malnutrition through community health volunteers during the three years of the project. Calculation of this indicator is revised in line with the definition and to exclude double counting. A total number of 18,180 children under 5 screened were screened by Community Health Volunteers (CG, iCCM), by nutrition outreaches in locations where community activities are not available and by some health facilities where IDP populations fluctuate and where community screening may not cover the whole catchment population. Medair requests to amend the target for this indicator to 27,478 (70% of total estimated children under 5 in project area) to demonstrate an increase in screening coverage as the project progresses. In 2025, a total of 56,136 children U5 were screened for malnutrition at all supported fixed HFs. During 2025 Medair support reduced from 12 to 8 HF due to reduced US funds. Medair handed over 1HF in Bardherre in Feb, and 1HF in Kismayo and 2 HF in Balcad at the end of August. In Q4 Medair introduced a new screening tool in the community to remove double counting. A total of 21,937 children were screened between Oct-Dec across all supported sites, of which 11,468 (5,66 girls, 5,982 boys) were unique beneficaries. Subtheme Nutrition Proportion (%) of the total number of discharged [as cured + defaulters + death] across all treatment facilities, over the period of programme which are discharged as cured. Year 1- 8 Whats Up parents (4 groups in Deynile, 2 Groups Nageyle and 2 groups in Karaan Districts) each composed of 15 participants (10 sessions of about 2hours each per target group) the facilitators will take the group participants through several topics such as: basics on child development, parenting skills and aware of social emotion developments). Year 2:4 Whats Up Ladies (2 groups in Caawiye-Deynile and 2 Groups Karaan(1Jamuriya and 1 Nageyle) consisting of 20 teenage ladies each (10 sessions of about 2hours each per target group) the facilitators will take the group participants through several topics such as:awareness on risk and safe areas in the community, causes of S/GBV and consequences, how to deal with emotions. Year 3: 4 Whats Up Children (2 groups in Caawiye-Daynile and 2 groups Karaan(1Jamuriya and 1 Nageyle) consisting of 15 Children ( 7-14 years) the facilitators will take the group participants through several topics such as:how to deal with emotions and cope with psychosocial problems resulting from many challenges eg conflict, abuse, separations The project successfully met its Year 1 targets by implementing 120 What Up Parenting sessions for 8 groups (4 in Caawiye, 2 in Negeyle, and 2 in Jamhuuriya). The selected participants attended 6 modules, consisting of 8 sessions, aimed at increasing parents' knowledge on managing children's emotions, nutrition, health, hygiene, and early childhood development (ECD). ( 2 sessions Following sessions were achieved. Session 1: Being a parent, The past, present and future Session 2: Growing up, Emotions Session 3: Dealing with difficulties Session 4: Understanding stress and supporting my child when under stress Session 5: Importance of play, age groups, interaction, creative plan material Session 6: Keeping children healthy Session 7: Social emotions support Session 8: Prevention and Homecare. Due to cultural and regious sensitiity, 2 sessions with modules on sexual education for each group were omitted. The project team successfully conducted 10 weekly structured sessions for the What’s Up Ladies participants selected in 2025, enhancing community awareness and engagement in GBV prevention, emotional well-being and social interaction. In Y2 target for the What’s Up Ladies activity was fully achieved as planned, reflecting strong participation and consistent implementation throughout the reporting period. To ensure accessibility and effective learning, the team reviewed and translated the training manual into Somali and conducted a comprehensive training for facilitators to strengthen their skills in delivering the sessions. A total of 10 weekly structured sessions were conducted smoothly, following 5 modules: Module 1: Getting Started Module 2: Dealing with Emotions Module 3: Sexual Gender-Based Violence (SGBV) Module 4: Trust and Communication Module 5: What Can Be Done These sessions provided a safe and supportive environment for women to share experiences, build emotional resilience, and enhance their communication and coping skills. Subtheme Protection 1) This indicator covers response to survivors of GBV (women, men, girls and boys).2) Appropriate response is defined as the provision of comprehensive services/assistance based on a thorough analysis of the risks faced by persons. The protection outcome of the response must be well defined. Relevant SOPs (including functional Referral Mechanism) should be followed. 3) Use the activity description field to describe what services (medical, MHPSS, legal, security, etc.), as well as SOPs in place.4) Number of persons must be disaggregated by age and sex at reporting stage.5) If action also includes prevention activities, please add a custom indicator to reflect this. In case it concerns medical care to GBV survivors, please capture this under HEA-I8. Provision of appropriate and timely comprehensive services/assistance including pre-referral counseling (psychological support first) and medical first aid at three supported primary health facilities, and referral to other actors for Other specialized protection Service. Medair will also broaden the scope of life-saving GBV services by improving service quality in Daynile and Karaan districts through the establishments of Community protection commitees and protection caseworkers will be involved in the identification of GBV Cases, awareness raising on GBV issues in community gathering at household level and coordinate with protection caseworkers to identify GBV survivors, safe referals facilitation, follow up. In Y1 GBV target was set at 120, but this was surpassed, reaching 166, representing a 38.3% overachievement. This positive outcome indicates increased service utilization and heightened community awareness of GBV concerns since the lauch of the CPCs. The project needs to review the target set for this indicator. A total of 259 Gender-Based Violence (GBV) cases were admitted, including 126 cases involving survivors under 18 years. This represents a notable overachievement, surpassing the set target by 216%. The increase in case identification and reporting is largely attributed to strengthened community awareness interventions that have improved access to GBV services at health facilities. In addition, continuous sensitization, messaging, and training provided by case workers and Community Protection Committees have played a key role in facilitating community referrals and engagement. Given this exceptional level of achievement, the project intends to revise the targets for this indicator to ensure they remain realistic and aligned with the evolving context. However, the revised targets have not yet been finalized. The breakdown of reported cases is as follows: Rape Cases = 5 Sexual Assault = 16 Physical Assault = 53 Forced Marriage = 118 Denial of Resources, Opportunities, or Services = 31 Psychological / Emotional Abuse = 36 Subtheme Accountability The purpose of this indicator is to facilitate the operationalisation of protection mainstreaming and provide a way to measure the identification, implementation and monitoring of required corrective actions/measures. The indicator is therefore a process indicator rather than an outcome indicator. This means that the difference between the value at the beginning (baseline) and at the end of the action (target) is not be the focus Protection mainstreaming incorporated throughout multisector activities described. Subtheme Accountability Active participation of beneficiaries could lead to concrete proposals in programme design and/or on how to improve existing implementation. In the activity description/comment field, indicate how this input from beneficiaries is gathered (planned) and at mid/end-term stage, what input was received from beneficiaries and to which adaptations this has led. The project will carry out close follow-up and monitoring of feedback from communities and partners during implementation. Feedback will be obtained through the quarterly engagement meetings mentioned above, through the toll-free feedback hotline number, and throgh exit interviews carried out at supported health facilities. The observed input/feedback will be documented and reviewed on a monthly basis in order to make necessary adjustments in a timely manner. The project has so far made 2 adaptations based on feedback from the beneficiaries. The first one was when What's Up Parents group members who complained about the unfavourable 9am meeting times which conflicted with their daily household chores timings. The members thus requested that the meeting times be moved to 12pm noon to allow them finish up their household chores before coming for their What's Up Parents group sessions. This was gladly adapted by the project. The second one was also prompted by feedback from the What's Up Parents group members, who pointed out the long distances from their homes to the meeting venue and requested for support and faciltation to enable them to attend group activities - which was eventually adapted as well. Subtheme Protection In activity description field:1) Define type of participants (population/authorities/Armed Forces/Armed Groups, etc.).2) Describe subject of training. 3) At reporting stage, the number of participants must be disaggregated by age and sex. Training of 176 persons consisting of 17 partner HF staff, 30 partner school teachers, 80 CHVs,4 Protection case workers, 20 community protection committees,15 community leaders and 10 Medair staff. Participants will be trained on the GBV core concepts and basic principles of child protection to acquire a good understanding of their roles in ensuring safety and wellbeing of children and GBV Survivors in the catchment locations(Karaan and Deynile) Year 1 training for 4 protection case workers, 3 Whats Up facilitators, 20 community protection commitees, 15 community leaders by Medair Year 2 training for 10 Medair staff, 17 partner health staff 30 school teachers by Medair Year 3 training for 80 CHVs by implementing partner with Medair supervision. The project carried out capacity-building to 49 individuals (3 health facility in-charges 3 GBV nurses, 4 case workers, 3 What’s Up facilitators, 2 CFS facilitators, 19 CPC members and 15 community elders) against a target of 41 participants, exceeding the target by 14.6%. The training focused on basic GBV and child protection (CP) principles. The project has successfully achieved its training target in Y2. A total of 57 participants (32 females and 25 males) were trained in 2025 on GBV core principles, and child protection. Of these, 10 were MEDAIR staff, 17 were health facility staff, and 30 were community schoolteachers. Subtheme Shelter and settlements Basic, safe and dignified: - affected individuals have an minimum covered floor area of 3.5m2 per person. - all shelter solutions and materials meet locally agreed technical and performance standards and are culturally acceptable. - all household (re-)construction is in accordance with safe building practices and standards. - all household (re-)construction demonstrate involvement of the affected population and/or are culturally acceptable by the affected population. Under Shelter (provision of plastic sheets), DAWA reached 706 individuals from 100 HHs in Beledweyne district. Zamzam reached 1,777 individuals from 280 HHs in Garasbaley district. SAACID reached 3,684 individuals from 390 HHs in Jilyaale IDPs settlements (Jowhar district) The total number of individuals reached through Crisis Modifier shelter intervention by the 3 partners is 6,167 individuals from 770 HHs. Subtheme Shelter and settlements Distribution of Non-Food Items. All household materials excluding those that fall under hygiene/dignity kits or direct shelter items. Also excludes unrestricted cash, which is to be put under indicator CSH-I1 or CSH-I2. However, if cash is provided for the purpose of buying household items, it should be put under this indicator (i.e. restricted cash). In 2025, Crisis Modifier (CM) funds enabled rapid assistance to 1,762 crisis‑affected households (HHs) across Garabaley and Gubadley Districts in Banadir Region, Mogadishu, and Jowhar District in Middle Shabelle. The interventions focused on providing essential non-food items (NFIs)—excluding hygiene and dignity kits to address urgent shelter and basic household needs among newly displaced and highly vulnerable IDP populations. Through CM activation, three implementing partners delivered timely, life‑saving support. Zamzam assisted 300 households in Garabaley with emergency shelter materials, including tarpaulins, ropes, and tent pegs, enabling affected families to quickly establish temporary shelters. In Jowhar District, SAACID reached 1,203 households with plastic sheets and long-lasting insecticide-treated mosquito nets (LLINs), addressing both emergency shelter gaps and malaria prevention needs. In Gubadley District, DAWA supported 259 households with tarpaulins and kitchen sets to restore essential living conditions. Subtheme WASH Hygiene promotion/awareness raising activities. Could be training, campaigns, radio broadcasts, flyers etc. Excludes the distribution of Hygiene/dignity kits that should be covered under WHS-I1. Using the Care Group concept with 545 CG volunteers (120 Jamhuriya, 245 Negeley in Karan, and 180 in Dayniile). SOMJR will fund 365 CG volunteers in Jamhuria and Nageyle, while the remaining 180 CG volunteers in Dayniile will be funded through other grants. Each volunteer will visit 15 neighbour households on a bi-weekly basis providing messages to all household members present. The volunteers will conduct hygiene promotion messaging using CHWs MoH harmonized guideline and the Somali translated Educational and Communications (IEC) booklets also in community forums (where possible). Monthly incentives will be done through the implementing partners (DAWA and ZAMZAM during the 3 year project period. One time incentive in year 1 will be carried out by Medair and implementing partners and in year 2 and 3 will be carried out by the implementing partners. Refresher trainings for the volunteers will be conducted by Medair and implementing partners in year 1 while in year 2 and year 3 trainings will be conducted by the partners (DAWA-Negeley and ZAMZAM -Jamhuria-Karan district) The project's 365 active Care Group Volunteers in Karan Nageyle and Jamhuriya provided key messages throughout the reporting period, with the highest number of unique individuals reached being 49,527 individuals (21,293 males and 28,234 females). Out of this number, 57 were PWDs. An additional 833 people received hygiene promotion during hygiene kit distributions. This gives a total of 50,360 as the number of people reached with hygiene promotion/awareness raising activities in Y1 of the project. In 2025, a total of 51,304 individuals were reached, including individuals who had also been supported in 2024. Of this total, 48,135 individuals were reached with hygiene promotion activities delivered through the same Care Group volunteers supported communities in 2024, ensuring continuity of behavior‑change messaging and household‑level hygiene reinforcement. In additional 1,666 individuals were reached through hygiene promotion implemented by the local partner Zamzam. In addition, 1,503 individuals were reached through the Crisis Modifier mechanism, including 300 individuals in Garasbaley reached by Zamzam and 1,203 individuals in Jowhar reached by SAACID, providing rapid support to households affected by emerging shocks. Subtheme Health Dimensions of mental health and/or wellbeing include ((as per IASC MHPSS Framework): 1) improved functioning (e.g. the ability to carry out essential activities for daily living); 2) improved subjective wellbeing (e.g. positive emotions, self-concept, life satisfaction, hopefulness); 3)Reduced distress and/or mental, neurological or substance use disorders (or symptoms thereof); 4) Improved ability of people with mental health and psychosocial problems to cope with problems (e.g. through skills in communication, stress management, problem-solving, conflict management or vocational skills); 5) Improved social behaviour (e.g. reduced aggression); 6) Improved social connectedness (e.g. increased quality and number of connections an individual has with other people in their social circles of family, friends and acquintances) To be measured at target group level. This indicator will measure the reported improvement in well-being among those who receive MHPSS as part of the SGBV care pathway. Related activities are described under HEA-I2 and HEA-I8 A total of 86 individuals receiving mental health and psychosocial support (MHPSS) services were assessed using the WHO-5 Well-Being Index at both baseline (first consultation) and follow-up (second consultation). The WHO-5 is a standardized tool designed to measure subjective psychological well-being, with scores ranging from 0 to 25. Results indicate that 100% of participants demonstrated improvement, defined as an increase in their WHO-5 score between the two time points. Statistical analysis using a one-sided paired-samples t-test confirmed a significant increase in well-being following the intervention. The overall mean score rose from 7.50 at baseline to 23.30 at follow-up, resulting in a mean difference of 15.80 points. This change was statistically significant (t(85) = 37.11, p < .001), with a very large effect size (Cohen’s d = 4.00), indicating a substantial and clinically meaningful impact of the MHPSS services on participant well-being. All health facilities showed consistent and significant gains. At Jamhuriya, the average score increased from 8.56 to 23.83, with a mean difference of 15.27 points and a corresponding effect size of 6.20. Similarly, participants at Negayle experienced a mean score increase from 6.16 to 22.63, yielding a mean difference of 16.47 points and an effect size of 3.15. In both cases, p-values were less than .001, confirming strong statistical significance. Subtheme WASH Regular and timely access: 250g soap/p/m for personal hygiene, 200g soap/p/month for laundry. This includes hygiene/dignity kits. Access may be in-kind or though voucher distribution or through unconditional restricted cash to enable beneficiary to buy soap (without compromising access to other basic needs). Distributed items need to be culturally acceptable i.e. take into account local practice and expectations. Distribution of Hygiene kit to 15,000 IDPs in Daynile District during emergencies including disease outbreaks. A total of 2500 Hygiene Distributions (833 Hygiene Kits per year). The Hygiene Kits distribution will be carried out in Daynile x 3 times during the project period by the local implementing partner (ZAMZAM) Post distribution monitoring for hygiene kits and Soap for hand demonstration will be carried after each distribution by Medair and implementing partner. Each household will receive 1) 800grms of soap 2) 2 (20L) jerrycans 3) 100 tabs of Aqua tabs Distribution of 200mg of soap for handwashing demonstration will be distributed to neighbour households in Karan through the 345 care group volunteers (120 in Jamhuriya through ZAMZAM and 245 in Negayle through DAWA) on a quarterly basis Medair reached 5,074 people through the distribution of WASH Non-Food Items (NFIs) including, 1600gm of soap, 2 pcs of 20L jerrycans and 10 strips of Aquatabs per household. Medair's partners reached an additional 6,181 people with hygiene kits as part of Crisis Modifier responses. Additionally, the project distributed 200-gram bars of soap to 5,475 households through the Care Groups. In 2025, a total of 25,301 individuals were reached through the distribution of WASH Non‑Food Items (NFIs) across the project locations. Of these, 9,865 individuals received NFI hygiene kits, primarily targeting new arrivals in the Garasbaley district. Each household received 1,600 grams of soap, two 20‑liter jerrycans, and 10 strips of Aquatabs, with the distribution carried out by the local partner Zamzam. A further 9,961 individuals were supported through the crisis modifier, including 1,592 individuals in Garasbaley reached by Zamzam and 8,369 individuals in Jowhar reached by SAACID, ensuring timely access to essential hygiene items and water treatment supplies for newly displaced and crisis‑affected households. Additionally, the project continued supplying 200‑gram bars of soap to the same 5,475 households supported in 2024 for Care Group Mothers Subtheme WASH Access: maximum distance to water point 500m, queuing time less than 15min, filling time maximum 3 min/20 litres or as locally agreed. Water access should be during the whole period of implementation unless action is mostly related to building the local water delivery capacity. Sufficient: covering basic needs, i.e. 7.5-15 l/p/d or as locally agreed. Safe: 1) low risk of faecal contamination, 2) No faecal coliforms detectable in any 100-ml sample, 3) For piped water supplies, or for all water supplies at times of risk or presence of diarrhoea epidemic, water is treated with a disinfectant to achieve free chlorine residual at the tap of 0.5 mg per litre and turbidity is below 5 NTU, 4) If for a short period, water which is contaminated chemically or radiologically is used, no (significant probability of) negative health effect is (likely to be) detected. Domestic use: drinking, cooking and personal hygiene (incl. laundry) To reduce walking distance to water sources and to improve access to safe drinking water, Medair will continue providing water to IDPs in Daynile at 9 water kiosks (44 taps) to serve approximately 2,730 households (16,380 individuals) in Daynile IDP settlement through implementing partner (ZAMZAM) every year during the 3 year project period To ensure safety during evening hours, installation of solar lighting at the 9 water kiosks and minor repairs for water kiosks aprons, drainage canals, replacement of the none functional/broken taps will be carried out by Medair during the 3 year project period Refresher training for the water user committees will be carried out by ZAMZAM every year during the 3 year project period. During the first year of the project, Medair rehabilitated 9 kiosks by installing solar lighting and improving aprons, drainage, sewage systems, and taps. These kiosks are now being operated by the Zamzam Foundation, which is now successfully delivering clean water to 16,380 individuals in the Daynile IDP settlements through the 9 kiosks fitted with 44 taps. Additionally, Zamzam Foundation trained 9 Water User Committees (WUCs) to manage the kiosks effectively. During the second year of the project, 11 Medair -rehabilitated kiosks were operated by Zamzam in the IDP settlements with Water User Committees supporting. Subtheme Health Health facility rehabilitated: - physical conditions of the facility prevents/limits the spread of nosocomial infections - the facility is safe (e.g. against natural hazards) - WASH services available respecting standards - storage conditions for medical supplies are according to norms. - Medical waste is disposed of according to standards. Use this sub-sector and indicator only if the project contributed substantially to the rehabilitation. Support for minor health facility rehabilitation(fences, roof, windows and doors) and maintenance of waste management facilities, water piping systems and storage, handwashing facilities, and painting and maintenance of latrines at 2 health facilities in Karan District and 1 health facility in Daynile District (based on the specific needs of each facility) by Medair during the 3 year project period The new Jamhuriya Health Facility in Karan District underwent rehabilitation in Q4, including the renovation of five rooms, waiting areas, a nutrition screening room, two latrines, a small store room, and the perimeter wall, enhancing its overall functionality and safety. Rehabilitation works in the other 2 facilities (Negeyle in Karan district and Caawiye in Daynile district) were also carried out and reported in Q3 report, bringing to 3 the number of supported health facilities that underwent rehabilitation works. During Quarter 4, the following rehabilitation and improvement works were successfully carried out at Jamhuria Health Facility and Negeyle Health Facility to enhance service delivery, hygiene, and overall functionality. Jamhuria Health Facility • Constructed two new additional rooms to increase space and improve service provision. • Expanded the outpatient department (OPD) room to accommodate a higher number of patients and improve patient flow. • Constructed an additional latrine dedicated for health facility staff use, improving sanitation and staff welfare. • Constructed two handwashing stations to promote infection prevention and control practices. • Rehabilitated the maternity room latrine to ensure a safe, clean, and functional sanitation facility for maternity clients. Negeyle Health Facility • Removed debris from the ash pit and safely disposed of it in a newly constructed pit fitted with a concrete lid to ensure proper waste management. • Rehabilitated the placenta pit and replaced it with a new secure concrete lid, improving environmental health and safety standards. • Rehabilitated four handwashing pipes, restoring functionality and supporting hygiene and infection prevention measures. Caawiye health facility • Rehabilitated two water kiosks to restore functionality and ensure reliable access to safe water for patients, staff, and the surrounding community. • Installed solar-powered lighting systems at the water kiosks to enhance visibility, improve safety during evening hours, and support continuous access to water services. constructions and rehabilitations conducted Quarter 1 to Quarter 3 reported to respective quarterly reports Subtheme Health Number of psychological and/or psychiatric consultations provided by a skilled provider. PSS for SGBV cases will also be provided. Medair will train 6 healthcare staff on mhGAP-HIG and ensure referral linkage to primary healthcare facilities and mental health specialists. In the first year of the project, 666 mental health consultations were recorded at the supported health facilities using mhGAP-HIG and Somalia national treatment guidelines. This represents 58% of the three-year project target of 1,147 consultations, the high turn out for the consultations is attributed to continuous awareness sessions of communities through the community health volunteers. The majority of MH consultations were for acute stress (63%), followed by epilepsy (14%), depressive disorders (7%), grief (7%), psychosis (6%), harmful substance use (3%), and intellutual disabilty (1%). The project continued delivering mental health consultations at supported health facilities in 2025, reaching a total of 358 consultations (286 being adults and 72 being individuals below the age of 18 years) in line with mhGAP-HIG and national treatment guidelines.
