Specifically, the project aims to: 1. Map and characterise the mobility-related policies across relevant sectors and organisations in selected cities in African countries. 2. Conduct a scoping review of scholarship on how contextual factors, actor relationships, and power dynamics shape the development and implementation of mobility-related policies in different urban contexts in the majority world. 3. Engage decision-makers to identify and understand their context-specific evidence needs for policy changes and implementation challenges to advance mobility. 4. Develop a partnership to foster capacity enhancement for interdisciplinary social science research on public health policy in select African university settings. 5. Widen partnerships and produce a proposal for a longer-term grant that develops a consortium approach to enhance expertise, knowledge and resources for political analyses, policy advice and influence at partner institutions to support mobility in African cities. The overall objective is to strengthen the policy coalition and enhance the capacity for planning and implementation of Uganda's national community health strategy (NCHS). By mobilizing knowledge and evidence, emphasizing community engagement, and developing actionable policy tools, the project will co-produce plans and frameworks that integrate routine service delivery with outbreak management. This research activity will provide insights to foster a more resilient, equitable, and responsive community health system (CHS) capable of addressing both everyday resilience and emergency health needs. Workstream 1 (building a cohesive policy community) aims to identify and characterize the actors, roles, incentives, power relations, institutional arrangements and collaboration networks shaping community health programming (CHP) and outbreak management (OM). This will be achieved through policy and program document review; 25–30 key informant interviews (KIIs); one small-group workshop; five stakeholder dialogue workshops; Political Economy Analysis (PEA); Social Network Analysis (SNA); and participatory causal-loop mapping through four workshops involving 25–30 participants each. Desktop outbreak simulation drills will also be conducted in three districts, involving 20–30 participants per site. Key deliverables will include a stakeholder map, political economy and network analyses, system maps, stakeholder engagement strategies, validated outbreak scenarios, and research/policy briefs. Findings will provide Workstream 3 with governance, stakeholder and system assumptions for modelling, while informing Workstream 2 on relevant CHW roles and operational actors. Workstream 2 (knowledge mobilization and capacity building to strengthen evidence-informed planning) aims to consolidate evidence and generate empirical knowledge on CHW roles, task mix, workload and resource needs during routine service delivery and outbreaks, while strengthening capacity for evidence-informed planning. This will be achieved through evidence landscaping and rapid synthesis; review of CHW activity logs/diaries and structured audits involving up to 200 CHWs; and in-depth interviews with participating CHWs. Capacity needs assessment will involve 60–70 district health teams, supervisors and frontline implementers, followed by training workshops/webinars involving 40–60 participants and reflective interviews with 15–20 participants. Key deliverables will include a consolidated evidence base, assessment of CHW workload and resource needs, identified capacity gaps, training products, policy briefs and knowledge-translation tools. These findings will inform Workstream 3's model assumptions and parameters, while Workstream 3 outputs will support subsequent training and knowledge mobilization. Workstream 3 (modelling and decision analysis to optimize outbreak preparedness and resource allocation) aims to integrate findings from Workstreams 1 and 2 into practical models and decision-support tools for outbreak preparedness, response and resource allocation. This will be achieved through participatory modelling, disease-transmission modelling, constrained optimization/linear programming, scenario simulation and sensitivity analysis, drawing on epidemiological, health-system and stakeholder-generated data. Participatory model-development workshops will involve 20–30 stakeholders, while 15–20 technical KIIs will explore preparedness planning and modelling requirements. Alternative outbreak scenarios will assess CHW deployment, response strategies and resource allocation, including effects on cases, deaths, health-system demand and DALYs. Key deliverables will include an integrated transmission and health-system model, resource-allocation analyses, simulated response strategies and an R-Shiny decision-support tool, validated with 15–25 district and national planners.
